DC—CIK细胞免疫治疗联合化疗对中晚期卵巢上皮细胞癌患者免疫功能的影响  

Effects of DC -CIK immunotherapy combined with chemotherapy on the immune function of ovarian epithelial carcinoma patients at medium - advanced stage

摘　　要：目的探讨树突状细胞（DC）-细胞因子诱导的杀伤细胞（CIK）细胞免疫治疗联合化疗对中晚期卵巢上皮细胞癌（OEC）患者外周血T细胞亚群、细胞因子分泌的影响，并评估其治疗效果。方法将49例中晚期OEC癌患者分为2组，其中23例采用DC—CIK联合Pc方案（紫杉醇＋卡铂）化疗为联合治疗组，26例同期采用单纯PC方案化疗为单纯化疗组。分别采用流式细胞术和液相多重蛋白定量（CBA）法检测患者治疗前后外周血淋巴细胞亚群和血清中细胞因子水平变化，并评价2组患者治疗后的不良反应、生活质量、生存期。结果3个周期治疗后，联合治疗组患者外周血中CD3＋T细胞、CIM＋T细胞、CD4＋／CD8＋比值、NK细胞比例及Thl细胞因子IFN-Y 、IL-2的水平与单纯化疗组比较，明显升高（P〈0．05），而CD8＋T细胞、CD4＋CD25＋Treg细胞比例及Th2细胞因子IL-4、IL-6、IL-10的水平与单纯化疗组比较，明显下降（P〈0．05）；Karnofsky评分与单纯化疗组比较，明显升高（P〈0．05）。2组阃不良反应发生率以及治疗后1年生存率差异无统计学意义fP〉0．05）。结论DC—CIK细胞免疫治疗联合化疗能够提高OEC患者机体的免疫功能，提高患者的生活质量，且具有较高的安全性。Objective To explore the effects of dendritic cell - cytokine induced killer cell （ DC - CIK） immuno- therapy combined with chemotherapy on the level of peripheral blood T lymphocyte subsets and eytokines in ovarian epi- thelial carcinoma （ OEC ） patients at medium - advanced stage and to evaluate the clinical efficacy. Methods A total of 49 medium - advanced OEC patients were divided into two groups ： a combined therapy group where 23 patients received both DC - CIK immunotherapy and paclitaxe ＋ carboplatin （PC） chemotherapy, and a monotherapy group where another 26 patients underwent PC chemotherapy alone. Then, the changes of peripheral blood T lymphocyte subsets and cytokines were examined by flow cytometry and cytometric bead array （CBA）. Meanwhile, both groups were compared for adverse reactions, quality of life and survival rate. Results After 3 treatment durations, compared with the monotherapy group, the combined therapy group presented remarkable increases in the number of CD3 ＋ and CD4 ＋ T cells, the ratio of CD4 ＋/CD8 ＋ , NK cell proportion and the levels of Thl cytokines IFN - -,/ and IL - 2 in the peripheral blood （ P 〈 0.05 ）, but marked decreases in the proportion of CD8 ＋ T cells and CD4 ＋ CD25 ＊ Treg cells, and the levels of Th2 eyto- kines IL - 4, IL - 6 and IL - 10 （ P 〈 0.05 ）. The Karnofsky score was significantly higher in the combined therapy group than the monotherapy group （P 〈 0.05 ）. No statistical difference was found as to the incidence of adverse reactions and 1 - year survival rate between both groups （P 〉 0.05 ）. Conclusion DC - CIK immunotherapy combined with chemo- therapy can enhance the cellular immune function and quality of life in OEC patients, with higher safety.
