足月儿呼吸窘迫综合征的4年回顾性分析

摘　　要：目的对我院近4年收治的足月儿NRDS资料进行回顾性分析,探讨其变化趋势.方法收集2012年1月至2015年12月住院的足月儿NRDS的一般临床资料、治疗方法、并发症及预后资料.采用SPSS16.0软件进行统计学处理,计数资料采用c2检验或Fisher’s精确检验,以p〈0.05为差异有统计学意义.结果（1）近四年共收治足月儿NRDS 194例,其中Ⅲ~Ⅳ期RDS 109例,按年份分两组：A组（2012年和2013年）及B组（2014年和2015年）.（2）治疗方法：42例应用PS,B组使用比例明显增高.（3）并发症：38例（19.6%）发生肺气漏,31例（16.0%）发生PPHN,18例（9.3%）发生VAP;VAP的比例B 组有下降.（4）预后：治愈 172 例（88.7%）,好转 16 例（8.2%）,死亡 12例（6.2%）.死亡率B 组有下降.结论4年来我院足月儿NRDS 的总数呈下降趋势,但重症的比率仍在上升,严重并发症如PPHN、肺气漏等并未下降,随PS 的应用逐渐增加,其总体预后较好,死亡率低.ObjectiveThis study was to investigate the NRDS in full-term neonates in recent four years, with the aim of analysis the changing trends of full-term neonate RDS. Methods All the full-term neonates admitted to the neonatal intensive care unit of The First People’s Hospital of Neijiang between January, 2012 and December, 2015 on the basis of RDS were analyzed. The clinical signs, laboratory data, X-ray, mechanical ventilation, complications and prognosis were collected. Data analyses were conducted using SPSS 16.0. Significances between differences were assessed using c2 or Fisher’s tests. A P value 〈0.05 was considered statistically significant. Results（1） There were 194 cases in four years, including 109 cases of grade Ⅲ -Ⅳ, which account for 56.2%. Patients were divided into group A（2012 and 2013） and group B（2014 and 2015）. There were 126 cases in group A and 68 cases in group B. （2） There were 42 cases using surfactant, which more in group B. （3）There were 38 cases（19.6%）had air leak, 31 cases（16.0%）had PPHN and 18 cases（9.3%）had VAP. The incidence of VAP had decreased in group B. （4） The mortality had decreased in group B. ConclusionThe population of full-term RDS had decreased, severe RDS still account for high proportion. Serious complications such as PPHN, pulmonary air leakage did not decline. The overall prognosis of full-term RDS is good, the death rate of it is low.
