子痫前期血浆多聚腺苷二磷酸核糖聚合酶及脂蛋白相关磷脂酶A2活性变化研究

摘　　要：目的探讨多聚腺苷二磷酸核糖聚合酶(PARP)及脂蛋白相关磷脂酶A2(Lp-PLA2)在子痫前期发病中可能的致病机制。方法选择2013年9月至2014年5月在中国医科大学附属盛京医院行剖宫产终止妊娠的妊娠晚期女性60例,根据是否存在子痫前期,将研究对象分为:子痫前期组32例(A组)以及正常孕妇组28例(B组)。采集孕妇肘前静脉血浆2 m L并应用酶联免疫分析法进行PARP及Lp-PLA2活性测量。结果 A组PARP、Lp-PLA2活性表达均显著高于B组(P<0.01)。PARP与Lp-PLA2活性呈正相关(r=0.234,P<0.01)。PARP活性与新生儿1 min Apgar评分、新生儿5 min Apgar评分存在相关性(r值分别为-0.243、-0.318,均P<0.05)。Lp-PLA2活性与孕妇收缩压、舒张压、脉压差、平均动脉压及新生儿5 min Apgar评分存在相关性(r值分别为0.342、0.335、0.245、0.351、-0.267,均P<0.05)。结论子痫前期患者体内存在PARP、Lp-PLA2的高活性表达,血浆PARP、Lp-PLA2可能参与了子痫前期的致病过程,且PARP及Lp-PLA2的高活性有可能对新生儿不良妊娠结局有一定预测作用。Objective To explore the possible pathogenic mechanism of poly ADP-ribose polymerase （PARP） and lipoprotein-associated phospholipase A2 （Lp-PLA2） in preeclampsia. Methods The activity of PARP and Lp-PLA2 in plasma of women in late pregnancy with preeclampsia （group A, n=32）, and normal control （group B, n=2g ） were mea- sured by enzyme linked immunosorbent assay （ELISA）.Results Compared with group B, the PARP activity and Lp- PLA2 activity in group A increasd （P〈0.01）, The level of PARP was positively correlated with the activity of Lp-PLA2 （r=0.234, P〈0.01 ）. The PARP activity was correlated with 1 rain apgar score and 5 min apgar score, with r value being- 0.243 （P〈0.05）and -0.318 （P〈0.05）, respectively. The Lp-PLA2 activity was correlated with systolic blood pressure, diastolic blood pressure, pusle pressure, mean artery pressure and 5 min apgar score, with r value being 0.342 （P〈0.05）, 0.335（P〈0.05） ,0.245（P〈0.05） ,0.351 （P〈0.05）and -0.267（P〈0.05）, respectively. Conclusion The PARP and Lp- PLA2 actvity increases in patients with preeclampsia, which shows that the PARP and Lp-PLA2 in plasma may be involved in the possible pathogenic mechanism in preeclampsia. The high actvity of PARP and Lp-PLA2 may imply the neonatal adverse pregnancy outcomes.
