摘　　要：目的加强病案回收管理质量,提高病案回收率,应用PDCA循环法构建病案回收长效管理机制。方法科室成立质量持续改进小组(CQI小组),遵循PDCA循环原则,以"提高病案回收率"为主题,开展为期6个月的质量持续改进活动,并将该次活动的预期目标值设定为"7个工作日内病案回收率≥90%",活动结束后继续监测4个月病案回收率情况,以确定PDCA循环法长效机制。结果遵循预定活动步骤,运用PDCA循环原理,医院病案按时回收率明显提高,7个工作日病案回收率从活动前的34.1%,提升到92.3%,达到预期目标值,活动成果非常显著。活动结束后,再延续监测4个月,7个工作日病案回收率依然保持90%以上,说明管理机制长期有效。结论运用PDCA循环的操作原理,可明显提高住院病案回收率,加强医师书写病历积极性,促进病案管理水平的提高。Objectives To enhance the quality of the medical record recycling management,improve the medical record recycling rate,apply PDCA circulation to structure the long-term management mechanism of medical record recycling. Methods Department set up quality management circle activities group,followed to the PDCA circulation principle,＆quot;Improving the recycling rate of medical records＆quot; was taken as the theme,carried up the quality management circle activities for 6 months,and set the expected goal of the event to ＆quot;more than 90% medical recycling within 7 working days＆quot;,kept monitoring the recycling rate of the medical records for 4 months after the event finished,so as to ensure the PDCA circulation long-term mechanism. Results Followed the steps of the quality management circle activities,applied the PDCA circulation principle,the hospital medical records on time recycling rate was obviously improved. The 7 days medical recycling rate increased from 34.1% to 92.3%,reached to the desired target value,the activity results were very significant. After the activities finished,kept monitoring for 4 months,the 7 days medical record recycling rate was still above 90%,which meant the long-term management mechanism was workable. Conclusions With the application of the PDCA circulation operation principle,we could obviously improve the recycling rate of hospital medical records,strengthen the activity of doctors writing medical records,and promote the improvement of the level of medical record management.
