重症肺炎患儿中万古霉素使用的药学监护

摘　　要：目的探讨重症肺炎患儿使用万古霉素的药学监护要点。方法选取我院2014年1月—2015年6月使用万古霉素的重症肺炎患儿30例,分析其中的典型案例,归纳总结临床药师参与重症肺炎患儿万古霉素治疗过程中的工作要点。结果临床药师对重症肺炎患儿万古霉素的监护可以从给药方案的制定、日剂量、给药频次调整,血药浓度异常波动分析,药物不良反应识别等方面去实施;药师与医师、护士有效沟通可以确保血液、痰液等取样准确性、提高给药的合理性和安全性。结论药师参与临床用药监护,可使万古霉素在重症肺炎患儿中的使用更加合理,能有效的识别及减少不良反应,发挥重要作用。Objective To investigate the role of pharmaceutical care in the use of vancomycin in children with severe pneumonia. Methods The 30 typical cases of severe pneumonia were selected in our hospital from Jan 2014 to Jun 2015, we summarized working points of vancomycin use in children with severe pneumonia by analyzing typical cases. Results Clinical pharmacists could monitor vancomycin use by designing regiments, daily dose adjustment, dosing frequency adjustment, analyzing the fluctuation of its plasma concentrations, and recognizing adverse drug reaction （ADR） of vancomycin. Effective communication between pharmacists and physicians, nurses could ensure the blood, sputum sampling accuracy etc, also improve the rationality and safety of the drug delivery. Conclusion The participation of clinical pharmacists in clinical treatment can promote the rational use of vancomycin in children with severe pneumonia and reduce the ADR, pharmacists play an important role in monitoring vancomycin use.
