血清瘦素、可溶性瘦素受体及沉默信息调节因子相关酶3与高血压左室肥厚的关系

摘　　要：目的:探讨血清瘦素(LP)、可溶性瘦素受体(SLR)及沉默信息调节因子相关酶3(SIRT3)水平与高血压左室肥厚(HLVH)之间的关系。方法:选择HLVH患者30例(HLVH组)、原发性高血压(EH)患者30例(EH组)以及健康体检者30例(对照组),行心脏彩超检查计算左室质量指数(LVMI),应用酶联免疫吸附法(ELISA)测受试者血清LP、SLR及SIRT3水平,同时检测空腹血糖(FPG)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、尿素氮(BUN)、肌酐(CR)等相关指标。结果:1EH组LVMI、血清LP及SIRT3水平高于对照组,血清SLR水平低于对照组(P<0.05),HLVH组LVMI、血清LP及SIRT3水平高于EH组和对照组,血清SLR水平低于EH组和对照组(P<0.05);2对于不同级别高血压病患者中,随着血压水平升高,患者LVMI及血清LP、SIRT3水平升高,血清SLR水平呈下降趋势(P<0.05);3LVMI与LP、SIRT3呈显著正相关(r=0.716,0.733;P<0.05),与SLR呈显著负相关(r=-0.675,P<0.05),以LVMI为因变量行多元线性逐步回归分析显示:SBP、LP、SLR、SIRT3、同型半胱氨酸(Hcy)进入多元线性回归方程。结论:EH及HLVH患者血清LP、SIRT3浓度升高,SLP浓度下降,并与血压之间有一定的相关性,说明LP抵抗、SIRT3与EH及HLVH的发生和发展密切相关。Objective：To analyze the relationship between serum leptin,soluble leptin receptor（SLR）and silent information regulator factor 2related enzyme 3（SIRT3）with hypertensive left ventricular hypertrophy（HLVH）.Method：Serum leptin,SLR and SIRT3 levels of 30 patients with HLVH（HLVH group）,30 patients with essential hypertension（EH group）and 30 healthy subjects（control group）were detected by enzyme-linked immunosorbent assay（ELISA）.All the subjects did UCG for LVMI measured fasting plasma glucose（FPG）,total cholesterol（TC）,triacylglycerol（TG）,high density lipoprotein cholesterol（HDL-C）,low density lipoprotein cholesterol（LDL-C）,blood urea nitrogen（BUN）and creatinine（CR）,etc.Result：LVMI and serum leptin,SIRT3 levels in EH group were significantly higher than those in control group,but lower than those in HLVH（P〈0.05）.Serum SLR in EH group was significantly lower than that of control group,but higher than that of HLVH group（P〈0.05）.In the patients with hypertension LVMI and serum leptin,SIRT3 levels were increasing and serum SLR was declining with elevated blood pressure levels.LVMI level was positively correlated with serum leptin and SIRT3levels（r=0.716,0.733,P〈0.05）and negatively correlated with serum SLR level（r=-0.675,P〈0.05）.Line to LVMI level as the dependent variable,multiple stepwise regression analysis showed that systolic blood pressure（SBP）,leptin,SLR,SIRT3 and homocysteine（Hcy）entered regression equation.Conclusion：The serum leptin,SIRT3 concentrations are increased,SLR concentrations are declined in patients with EH and HLVH,and correlate with blood pressure,indicating apossible role of leptin resistance and SIRT3 closely in the development and progression of EH and HLVH.
