摘 要:目的探讨在应用抗菌药物前,检测患者血液中细菌内毒素(ET)对诊断革兰阳性菌、革兰阴性菌及病毒感染的临床意义。方法选取2015年9月-2016年4月发热患者124例,将患者分为细菌组79例和病毒感染组45例,细菌组含革兰阴性菌45例、革兰阳性菌34例,分别测定两组患者血清内毒素含量和白细胞计数。结果通过秩和检验统计分析,革兰阴性菌患者的内毒素水平均大于革兰阳性菌患者和病毒感染者的内毒素水平,差异有统计学意义(P<0.05);革兰阴性菌患者的白细胞计数随内毒素水平的升高而升高(r=0.354,P<0.05)。结论感染患者在应用抗菌药物前,通过检测血浆内毒素水平可以初步判断患者感染的细菌种类,有利于指导临床抗菌药物的合理使用。OBJECTIVE To explore the clinical significance of detection of plasma endotoxin(ET)in diagnosis of gram-positive bacteria,gram-negative bacteria,and viral infections before the use of antibiotics.METHODS A total of 124 fever patients who were treated in the hospital from Sep 2015 to Apr 2016 were enrolled in the study and divided into the bacterial group with 79 cases and the viral infection group with 45cases;the bacterial group consisted of 45 cases of gram-negative bacteria infections and 34 cases of gram-positive bacteria infections.The serum endotoxin contents and white blood cell counts of the two groups of patients were determined.RESULTS The rank test and statistical analysis showed that the level of endotoxin of the patients with gram-negative bacteria infections was significantly higher than that of the patients with gram-positive bacteria infections and the patients with viral infections(P〈0.05).The white blood cell counts of the patients with gram-negative bacteria infection were increased with the level of endotoxin(r=0.354,P〈0.05).CONCLUSION The detection of plasma endotoxin level before the use of antibiotics may facilitate the preliminary identification of the species of bacteria and provide guidance for the reasonable clinical use of antibiotics.
