血清C-反应蛋白对儿童急性呼吸道感染的诊断价值研究

摘　　要：目的检测儿童急性呼吸道感染中血清C-反应蛋白(CRP)水平,评价其临床诊断价值。方法选取2014年9月-2015年9月就诊的急性呼吸道感染儿童122例,根据感染病原体不同将其分为细菌感染组58例与病毒感染组64例,另选取同期进行体检的健康儿童60例作为对照组;对三组儿童进行白细胞计数及血清CRP水平检测,比较各指标的差异。结果与病毒感染组、对照组比较,细菌感染组患儿就诊时血清CRP水平与CRP异常率均明显升高,差异有统计学意义(P<0.05);与治疗前比较,细菌感染组治疗后其血清CRP水平与CRP异常率均明显降低,差异有统计学意义(P<0.05)。结论血清CRP的检测有助于儿童急性呼吸道感染的早期鉴别与诊断,动态监测其水平变化在临床上具有一定的应用价值,可作为早期诊断儿童急性呼吸道感染的重要指标。OBJECTIVE To detect the level of serum C-reactive protein（CRP）in children with acute respiratory tract infections and evaluate the clinical diagnostic value.METHODS A total of 122 children with acute respiratory tract infections who were treated in hospitals from Sep 2014 to Sep 2015 were enrolled in the study and divided into the bacterial infection group with 58 cases and the viral infection group with 64 cases according to the pathogens causing the infections,meanwhile,60 healthy children who received the physical examination were set as the control group.The white blood cell counts and serum CRP level were determined and compared among the three groups of children.RESULTS The level of serum CRP and abnormal rate of CRP were significantly higher in the bacterial infection group than in the viral infection group and the control group during the treatment（P〈0.05）.The level of serum CRP and abnormal rate of CRP of the bacterial infection group were significantly lower after the treatment than before the treatment（P〈0.05）.CONCLUSIONThe detection of serum CRP may facilitate the early identification and diagnosis of the acute respiratory tract infections in the children,the dynamic monitoring of change of the serum CRP level has certain clinical value,and it can be used as an important indicator for early diagnosis of the children with acute respiratory tract infections.
