摘 要:目的探讨中枢神经系统感染患儿脑脊液及血白细胞计数(WBC)、降钙素原(PCT)、C-反应蛋白(CRP)变化及相关性,为儿童中枢神经系统感染的鉴别诊断提供参考。方法选取2014年1月-2015年12月住院的80例中枢神经系统感染患儿(感染组),另选同期需在硬膜外麻醉下腹股沟疝行疝修补术治疗患儿40例(对照组),采集血及脑脊液检测WBC、PCT、CRP,进行对比分析。结果感染组患儿脑脊液及血WBC、PCT、CRP均明显高于对照组(P<0.05),化脓性脑膜炎患儿脑脊液及血WBC、PCT、CRP均明显高于病毒性脑炎/脑膜炎患儿(P<0.05),病毒性脑炎/脑膜炎患儿脑脊液及血CRP均明显高于对照组(P<0.05);pearson相关分析中枢神经系统感染患儿脑脊液与血WBC、PCT、CRP间有正相关性(P<0.05),其中化脓性脑膜炎患儿脑脊液与血WBC、PCT、CRP有正相关性(P<0.05),病毒性脑炎/脑膜炎患儿CRP脑脊液与血间有正相关性(P<0.05),WBC、PCT无相关性。结论儿童中枢神经系统感染患儿脑脊液及血WBC、PCT、CRP具有明显相关性,检测以上指标可对化脓性脑膜炎与病毒性脑炎/脑膜炎进行鉴别,化脓性脑膜炎时WBC、PCT、CRP明显升高,病毒感染时CRP轻度升高但WBC、PCT无明显变化。OBJECTIVE To explore the changes of cerebrospinal fluid and blood white blood cell(WBC)counts,procalcitonin(PCT),and C-reactive protein(CRP)in children with central nervous system infection and analyze the correlation so as to provide guidance for differential diagnosis of the children with central nervous system infection.METHODS Totally 80 children with central nervous system infection who were hospitalized from Jan 2014 to Dec 2015 were chosen as the infection group,meanwhile,40 children who needed to receive inguinal hernia repair under epidural anesthesia were set as the control group.The blood and cerebrospinal fluid samples were collected to detect the WBC,PCT,and CRP.RESULTS The levels of cerebrospinal fluid and blood WBC,PCT,and CRP were significantly higher in the infection group than in the control group(P〈0.05).The levels of cerebrospinal fluid and blood WBC,PCT,and CRP were significantly higher in the children with purulent meningitis than in the children with viral encephalitis or meningitis(P〈0.05).The levels of cerebrospinal fluid and blood CRP were significantly higher in the children with viral encephalitis or meningitis than in the children of the control group(P〈0.05).The pearson correlation analysis showed that the cerebrospinal fluid and blood WBC,PCT,and CRP of the children with central nervous system infection were positively correlated(P〈0.05),there was positive correlation between the cerebrospinal fluid and blood WBC,PCT,and CRP of the children with purulent meningitis(P〈0.05),and there was positive correlation between the cerebrospinal fluid and blood CRP of the children with viral encephalitis or meningitis,while there was no correlation between WBC and PCT.CONCLUSIONThere is significant correlation between the cerebrospinal fluid and blood WBC,PCT,and CRP of the children with central nervous system infection.The detection of the above indicators may facilitate the differential diagnosis of purulent meningitis and viral encephalitis or meningi
