摘　　要：目的:通过对EV71型重症手足口病患儿的免疫学特点进行研究,探讨并分析患儿机体免疫功能与疾病的发生以及进展之间的关系。方法:以我院住院的EV71型手足口病患儿轻症与重症各20例作为研究对象,分别为轻症组与重症组,对其血清补体、血清免疫球蛋白以及外周血免疫细胞群进行检测。结果:重症组T细胞比例、CD3+、CD4+、CD8+表达水平相对于轻症组显著降低,而且CD4+/CD8+方面重症组也显著小于轻症组,P<0.05,差异具有统计学意义。B细胞比例、IgA、IgM、IgG表达水平显著升高,具有统计学意义(P<0.05),而C3、C4表达水平显著降低,也具有统计学意义(P<0.05)。预后进行多因素Cox回归分析:CD4、CD8、CD16、IgM、IgG等因素影响EV71型重症手足口病的生存及预后。结论:手足口病病情进展的危险因素一方面可能是T细胞功能受到抑制,另一方面可能是人体体液功能的亢进。CD4、CD8、CD16、IgM、IgG等各种因素对EV71型重症手足口病的生存及预后有一定的影响。Objective： Study by EV71 HFMD children with severe type of immunological characteristics, to explore and analyze the relationship between children and the occurrence of immune function and disease progression between. Methods：In our hospital HFMD children with mild to severe 20 patients as research subjects analyzed were severe group and mild group, their serum complement, serum immunoglobulins and immune cells were detected in peripheral blood. Results：Cell ratio severe group T,CD3^＋ , CD4^＋ ,CD8^＋ expression levels relative to the significant reduction in mild group, and CD4 ^＋/CD8^＋ aspects of severe group was also significantly lower than mild group, P〈0. 05, the difference was statistically significant. The proportion of B cells,IgA, IgM, IgG expression levels were significantly higher, with statistical significance （P〈0. 05）, and C3, C4 expression levels were significantly reduced, also statistically significant （P〈0.05）. Prognostic multivariate Cox regression analysis： CD4, CDS, CD16, IgM, IgG and other factors affecting the survival and prognosis of myasthenia EV71 HFMD. Conclusion：The risk factors for foot and mouth disease pro gression may be an aspect of T cell function is suppressed,on the other hand may be a function of body fluids hyperthyroidism. Various factors CD4,CD8,CD16, IgM, IgG, etc. have a certain effect on the survival and prognosis of myasthe nia EV71 HFMD.
