动脉血管重建术治疗血管性勃起功能障碍近远期疗效研究

摘　　要：目的：研究动脉血管重建术治疗血管性勃起功能障碍（ED）的近期与远期疗效。方法：选取2010年5月至2013年5月间入院诊治的动脉性ED患者98例。应用阴茎背深静脉动脉化手术治疗。术后应用电话、邮件、到院复查等方式对患者进行随访1～2年。观察比较患者手术前后IIEF-5（国际勃起功能）评分,并调查患者阴茎勃起能力。结果：术前患者IIEF-5评分仅为（7.18±1.05）分;术后3个月患者IIEF-5评分分别为（23.66±9.01）分,与术前差异显著（P〈0.05）;术后2年患者IIEF-5评分下降到（16.93±6.24）分,但仍与术前差异显著（P〈0.05）。术后3个月患者总有效率为91.84%,术后2年总有效率为72.45%。98例患者术后3个月完全勃起41例（40.82%）,总有效率94.90%;术后2年完全勃起22例（22.45%）,总有效率78.57%。结论：动脉血管重建术治疗血管性勃起功能障碍的近远期均较好,且患者阴茎勃起功能维持较高水平,但该手术需要注意适应症的选择。Objectives： To study the short and long -term effect of arterial revascularization for the treat- ment of vascular erectile dysfunction （ED）. Methods： 98 ED patients hospitalized between May 2010 and May 2013 were selected. All received deep vein dorsal penile artery surgery and were followed up for 1 - -2 years by telephone, mail and other ways. International Index of Erectile Function （ IIEF - 5 ） scores before and after surgery was compared, and the erectile capacity was investigated. Results： The preoperative IIEF- 5 score was only （7.18 ± 1.05 ） points, that 3 months after treatment was （23.66 ± 9. 01 ）, with significant difference （ P 〈 0.05 ）. The IIEF -5 score 2 years after decreased to （ 16.93 ±6.24）, with significant difference from preoperative score （P 〈 0. 05）. The total effective rate 3 months and 2 years after treatment was 91.84% and 72.45% respectively. Of the 98 patients, 41 cases （40. 82% ） had full erection 3 months later, a total effective rate of 94.90% ; 22 cases （22.45%） had full erection 2 years later, a total efficiency of 78.57%. Conclusion： The artery revascularization treatment of vascular erectile dysfunction is of good short and long - term effect, with high level of erectile function, but indications should be noted for surgery choices.
