真空小梁成形术的临床疗效和安全性临床分析

摘　　要：目的分析真空小梁成形术治疗原发性开角型青光眼与高眼压症的安全性与有效性。方法研究对象为80名患者（80眼）。观察真空小梁成形术后前房角、24h眼压、黄斑区视网膜神经节细胞复合体厚度及视野的变化。结果前房角开放范围：术后1周、1个月、3个月分别为（30．60±1．82）度、（29．77±1．27）度及（29．19±1．23）度，与治疗前的（28．78±1．24）度相比，差异均有统计学意义（P：0．00，P=0．00，P=0．00）。术后3个月与术前相比，办公时段、非办公时段平均眼压差异均有统计学意义（P=0．00，P=0．00）。术后3个月测量黄斑区平均、上方、下方视网膜神经节细胞复合体厚度与术前相比，差异均无统计学意义（P=0．30，P=0．95，P：0．28）。术后3个月平均视野缺损值及模式标准差与术前相比，差异均无统计学意义（P=0．57，P：0．66）。结论真空小梁成形术对于正确选择的患者是一种有效、安全、简单的治疗方法。Objective To analyse the efficacy and safety of pneumatic trabeculoplasty （PNT） for patients with primary open-angle glaucoma or ocular hypertension. Methods There were 80 eyes of 80 patients with primary open-angle glaucoma or ocular hypertension in our study. The anterior chamber angle, 24 hours intraocular pressure （IOP） , the thickness of macular ganglion complex and Humphrey visual field examinations after PNT were observed. Results The opening angles of anterior chamber angle were （28.78 ± 1.24） degrees before PNT, and （ 30.60 ± 1.82 ） degrees, （ 29.77 ± 1.27 ） degrees, （ 29. 19 ± 1.23 ） degrees at 1 week, I month, 3 months after PNT, and the differences were all statistically significant （P = 0. 00,P = 0. 00,P = 0.00）. The differences of IOP between before and after PNT at the out-office time and office time were statistically significant （ P = 0. 00, P = 0.00）. The average thicknesses of macular area, upper and lower retinal ganglional complex were detected and the differences were all not statistically significant（ P = 0. 30 ,P = 0. 95 ,P = 0.28 ）. The mean defect and pattern standard deviation of visual field at 3 months after PNT were compared with that before the surgery. The differences were not statistically significant （ P = 0. 57, P = 0.66 ）. Conclusion PNT is an effective, safe and simple method in the treatment of patients with glaucoma or ocular hypertension.
