中药肛滴治疗恶性肠梗阻的护理临床观察

摘　　要：目的：探讨中药肛滴治疗恶性肠梗阻的临床护理效果及操作流程。方法：将100例患者随机分为两组。标准组50例,常规组50例。采用将中药加热后,注入改良后的灌肠袋中,调节滴速（标准组肛管插入15~35cm,滴速为40滴/分;常规组肛管插入7~10cm,滴速为60滴/分）缓慢经直肠滴入。7天为1个疗程,每天2次,每次100ml。治疗期间禁食禁饮,胃肠减压,静脉营养支持,根据患者情况给予止痛、解痉药。观察治疗效果及护理操作流程化。结果：标准组总有效率96%,常规组总有效率78%,两组比较差异有统计学意义（P〈0.05）。结论：改良后的中药肛滴治疗恶性肠梗阻,具有中医药特色,操作简便、费用适中、治疗方案安全可靠、护理操作流程化等特点,易于患者接受,便于临床推广应用。Objective： To investigate the clinical nursing effect and operation flow of the treatment of malignant intestinal obstruction with traditional Chinese medicine anus drops. Methods： 100 patients were randomly divided into two groups. One was the standard group of 50 patients,the other was the conventional group of 50 patients. Injecting heated traditional Chinese medicine into the modified enema bag,then adjusted the dropping speed,（ group standard anal insertion 15 ~ 35 cm,dripping speed for40 drops per minute; group routine anal insertion 7 ~ 10 centimeter,dripping speed for 60 drops per minute） instill into the rectum slow. 7days for a course of treatment,2times a day,each time 100 ml. During the treatment,fasting,gastrointestinal decompression,intravenous nutritional support,giving analgesic,antispasmodic according to the conditions of patients. Observing of therapeutic effect and nursing operation flow. Results： The total effective rate of the standard group was 96%,the total effective rate was 78%,the difference was statistically significant between the two groups（ P〈0. 05）. Conclusion： Improved enema with traditional Chinese medicine for malignant bowel obstruction has the characteristics of traditional Chinese medicine,simple operation,moderate cost,reliable and safe in treating scheme,nursing operation flow characteristics and is easy to be accepted by patients,and is also easy for clinical application.
