基于现代名老中医经验的肺痿常见证候方药规律研究

摘　　要：目的总结现代名老中医诊治肺痿的辨证用药配伍规律。方法检索现代名老中医肺病数据库和期刊中医肺病数据库,提取纳入文献证候、方剂、药物等信息,建立现代名老中医诊治肺痿文献研究数据库,统计肺痿常见证候及治疗采用方剂(经方、时方)、中药使用频率,并对各常见证候所用药物进行系统聚类分析和二元相关分析。结果共纳入13篇期刊文献及33本著作。肺痿常见证候分别为肺气阴两虚证、肺阴虚证、痰热壅肺证、肺气虚证、肺胃阴虚证和痰瘀阻肺证。肺气阴两虚证常用方剂有麦门冬汤和炙甘草汤;常用药物有麦冬、甘草、半夏;常用药对有熟地黄与石斛、枇杷叶与石膏、枇杷叶与熟地黄。肺阴虚证常用方剂为麦门冬汤;常用药物有麦冬、甘草、南沙参;常用药对有生地黄与党参、麦冬与半夏、知母与化橘红。痰热壅肺证常用方剂为千金苇茎汤;常用药物有苦杏仁、川贝母、甘草;常用药对为竹茹与芦根、桔梗与枳壳、川贝母与枇杷叶。肺气虚证常用方剂为独参汤;常用药物为黄芪、白术;常用药对有黄芪与牡蛎、丹参与当归、太子参与白术。肺胃阴虚证常用方剂为现代名老中医自拟方;常用药物有茯苓、甘草、川贝母;常用药对有砂仁与莲子、半夏与化橘红、陈皮与南沙参。痰瘀阻肺证常用方剂为香附旋覆花汤;常用药物有半夏、党参、红花;常用药对为丹参与赤芍、丹参与桃仁、枇杷叶与百部。结论现代名老中医治疗肺痿常以气虚、阴虚、痰、瘀、热为中心辨证用药,各证候用药均具有一定规律和较高临床实用价值。Objective To summarize the experiences of modern famous veteran Chinese medicine doctors in compatibility rules of using herbs by syndrome differentiation in the diagnosis and treatment of lung wilt disease.Methods Database of Experiences of Modern Famous Veteran Chinese Medicine Doctor for Lung Disease and Journal Database of Chinese Medicine for Lung Disease were searched,and information,such as syndromes in papers included,prescriptions,herbs,etc. was extracted to build the Literature Database of Experiences of Modern Famous Veteran Chinese Medicine Doctors in Diagnosis and Treatment of Lung Wilt Disease. Common syndromes of Lung wilt disease,prescriptions（ classical prescriptions and current prescriptions） for treatment,as well as herbs using frequency were calculated. Systematic cluster analysis and two elements correlation analysis were conducted for herbs in treating each common syndrome. Results Common syndromes of lung wilt disease include syndrome of deficiency of lung Qi and Yin,syndrome of deficiency of lung Yin,syndrome of phlegm-heat obstructing lung,syndrome of deficiency of lung Qi,syndrome of deficiency of lung and stomach Yin,as well as syndrome of phlegm and blood stasis obstructing lung. Commonly used prescriptions for treating syndrome of deficiency of lung Qi and Yin were Maimendong Decoction and Zhigancao Decoction. Commonly used herbs were Maidong [Radix Ophiopogonis,麦 冬 ],Gancao [Radix liquiritiae,甘草] and Banxia [Rhizoma Pinelliae,半夏]. Commonly used couplet herbs were Shudihuang [Radix Rehmanniae Preparata,熟地黄]-Shihu [Herba Dendrobii,石斛],Pipaye [Folium Eriobotryae,枇杷叶]-Shigao[Gypsum Fibrosum,石膏]and Pipaye-Shudihuang. Commonly used prescription of treating syndrome of deficiency of lung Yin was Maimendong Decoction（ 麦门冬汤）. Commonly used herbs were Maidong,Gancao and Nanshashen[Radix Adenophorae,南沙参].
