摘  要:目的探讨便携式体外生命支持系统在失血性休克救治中的有效性与安全性。方法以小型巴马猪为实验对象,体质量(46.0±5.2)kg,共12只,雄性,建立失血性休克模型,肝素化后转流,应用低温脏器保护液逐步降温至15℃并停止循环。将动物随机分为两组:A组停循环时间为90 min;B组停循环时间为120 min,每组各6只。分别记录操作前后心率、平均动脉压、呼吸、体温、末梢氧饱和度及神经系统评分等。结果 A组死亡0只,存活6只(100%);B组死亡3只,存活3只(50%);两组存活率比较,差异有统计学意义(P<0.05)。术后1 d神经系统评分,A组明显高于B组(P<0.05);两组术后7 d比较,差异无统计学意义(P>0.05)。结论在失血性休克动物模型救治中,便携式体外生命支持系统可诱导深低温(15℃)水平停循环90 min,保证实验动物存活及神经系统功能正常,安全有效。Objective To investigate the effectiveness and safety of portable extracorporeal life support system in a swine model of of hemorrhagic shock. Methods Twelve swines were established as model of hemorrhagic shock. The models were induced profound hypothermia( 15℃) circulation arrest by hypothermic organ preservation solution. The models were randomly divided into 2 groups: Group A,hypothermia circulation arrest was 90 minutes,and Group B was 120 minutes. Heart rate,mean arterial pressure,respiration rate,body temperature,peripheral oxygen saturation and neurological score were recorded before and after operation. Results In Group A,0 case died,6 cases alive( 100%),while in Group B,3 cases( 50%) died and 3 cases( 50%) survived,and the difference was significant( P〈0. 05). Postoperatively,the neurological score in Group A was significantly higher than that of Group B( P〈0. 05) in the first day,while there was no significantly difference in the seventh day. Conclusion The models of hemorrhagic shock could be induced profound hypothermia( 15℃) circulation arrest for 90 minutes by the portable extracorporeal life support system. The machine ensured that the animals survival and neurological function normal,which was safe and effective.
