摘 要:目的探讨联合检测降钙素原(PCT)与C反应蛋白(CRP)对脓毒症患者的临床价值。方法选择2013年6月至2014年12月在鄂尔多斯市中心医院重症医学科(ICU)住院的细菌感染患者78例,随机分为(前面4个字删掉)其中脓毒症组42例、局部感染组36例,选取同期住院但无感染的患者20例为对照组。对患者血清PCT与CRP水平及动态变化进行检测分析。结果 PCT水平在脓毒症组、局部感染组、对照组分别为(15.47±9.22)μg/L、(4.21±0.56)μg/L、(0.62±0.25)μg/L,CRP水平在脓毒症组、局部感染组、对照组分别为(64.6±22.73)mg/L、(44.8±17.81)mg/L、(6.41±4.21)mg/L。脓毒症组和局部感染组血清PCT、CRP浓度均高于对照组(P<0.05);经抗感染治疗后,脓毒症组和局部感染组PCT、CRP水平均下降,与治疗前比较,差异有统计学意义(P<0.05);脓毒症组与局部感染组比较,差异有统计学意义(P<0.05)。结论联合检测血清PCT与CRP可为脓毒症早期诊断提供可靠的依据,动态检测PCT、CRP水平有助于临床判断病情变化及预后。Objective To evaluate the clinical values on combined determination of procalcitonin(PCT)and C- reactive protein(CRP) in patients with sepsis. Methods Seventy - eight patients with bacterial infection from June 2013 to December 2014 hospitalized in ICU of Erduosi Central Hospital were enrolled, they were randomly divided into sepsis group(n = 42), local bacterial infection group( n = 36), twenty noninfectious patients during the same time were recruited as control group(n = 20). Serum PCT and CRP were measured, their dynamic change and the results were analyzed. Results The PCT concentrations in sepsis group, local bacterial infection group and control group were(15.47 ± 9.22)/tg/L, (4.21 ± 0.56)mg/L, (0.62±0.25)mg/L, the CRP concentrations in sepsis group, local bacterial infection group and control group were(64.6± 22.73) mg/L, (44.8 ±17.81)mg/L, (6.41 ± 4.21)mg/L, respectively. The serum level of PCT and CRP in the two infection groups were all higher than the control group(P〈0.05). After the anti - infective therapy , The levels of PCT and CRP decreased in the two infection groups than before (P (0.05). There were statistical significances between sepsis group and local bacterial infection group(P〈0.05). Conclusions The combined determination of serum PCT and CRP could provide reliable evidence for early diagnosis in patients with sepsis. Dynamic detection of PCT and CRP level are helpful to the clinical judgment for the condition changes and prognosis of the disease.
