摘 要:目的观察丙泊酚靶控全身输注麻醉用于骨外伤危重患者急诊救治的效果。方法回顾性分析80例骨外伤危重患者的临床资料。根据麻醉方式不同分为观察组与对照组各40例。对照组行七氟烷吸入全麻;观察组行丙泊酚靶控全身输注麻醉。观察、比较2组麻醉相关指标(不同时间段心率、平均动脉压、恢复自主呼吸时间、睁眼时间和应答时间等)与麻醉不良反应发生率。结果 2组诱导后心率、平均动脉压均明显低于本组诱导前,但2组间各时间点心率、平均动脉压比较差异无统计学意义(P>0.05);观察组恢复自主呼吸时间、睁眼时间、应答时间均短于对照组(P<0.01);观察组麻醉不良反应总发生率为7.5%,低于对照组的10.0%,但2组比较差异无统计学意义(P>0.05)。结论丙泊酚靶控全身输注麻醉用于骨外伤危重患者急诊救治的麻醉相关指标优于吸入全麻,且不良反应小,值得临床推广应用。Objective To observe the effect of propofol target controlled infusion anesthesia for emergency treatment of critically ill patients with bone trauma. Methods Retrospective analysis of clinical data of 80 patients with severe bone trauma. Divided into observation group and control group according to the different anesthesia methods, each of the 40 cases. The control group was given sevoflurane inhalation anesthesia, the observation group received propofol target controlled infusion anesthesia. Comparison of anesthesia related indexes of 2 groups ( different time heart rate, mean arterial pressure, recovery of spontaneous breathing time, eye opening time and response time) and the incidence of adverse reaction of anesthesia. Results The heart rate and mean arterial pressure in the 2 groups were significantly lower than that before induction, and there was no significant difference in the rates of snacks and mean arterial pressure (P 〉0.05 ) ;The observation group recovered spontane- ous breathing time, eye opening time, response time is shorter than the control group ( P 〈 0. 0l ) ; The total incidence of adverse reactions in the observation group was 7.5% ,lower than that of the control group (10%) ,but there was no significant differ- ence between the 2 groups ( P 〉 0.05 ). Conclusion Propofol target controlled infusion anesthesia for emergency treatment of critically ill patients with bone trauma, anesthesia related indicators are better than inhaled anesthesia, and adverse reaction is small, it is worth clinical application.
