河南省2015年MSM人群艾滋病检测咨询情况分析

摘　　要：目的掌握河南省男男性行为人群艾滋病咨询检测情况及HIV与梅毒感染状况,为MSM人群艾滋病防治策略制定提供科学依据。方法应用艾滋病综合防治信息系统检测咨询数据,分析MSM人群社会人口学特征以及HIV和梅毒感染状况。结果 3157名MSM人群接受了HIV抗体检测,HIV筛查阳性626人,筛查阳性率19.83%;梅毒检测2615人,阳性98人,阳性检出率3.61%。20岁以下、40-49岁以及50岁以上MSM人群HIV筛查阳性率较高,分别为23.9%、29.4%和53.2%。相对于初中及以下文化程度人群,高中/中专、大专以上人群HIV阳性检出率较低（OR 95%CI：0.56-0.86;0.60-0.93）。疾控机构检测人群HIV筛查阳性率高于医疗机构（OR=2.06,95%CI：1.48-2.88）。梅毒抗体阳性者HIV筛查阳性率更高（OR=4.68,95%CI：3/11-7.04）。结论河南省MSM人群HIV筛查阳性率处于较高水平,20岁以下及40岁以上年龄,已婚人群和丧偶人群以及初中以下文化程度者是MSM人群中HIV感染的主要危险人群,对MSM人群干预应针对这些特点人群重点开展。Objective To investigate HIV consultant and test status among Men who have sex with Men （MSM）, and get information about HIV and syphilis infection among MSM, provide scientific evidence for AIDS prevention and control policy development. Methods Use data from National information system on AIDS prevention and control, consultant and HIV test data about social demographic characteristics, HIV and syphilis infection among MSM was analyzed. Results Totally 3157 MSM received HIV consultant and test, among which 626 MSM test HIV antibody positive with a prevalence of 19.83% ; 2615 MSM test syphilis with a prevalence of 3.61%. MSM aged under 20years, between 40 to 49 years and beyond 50 years have a higher prevalence of HIV infection （23. 9% ,29.4% and 53.2%, respectively. ） MSM with high school, college education have lower HIV positive rate than those with junior middle school education and below （OR 95% CI：0. 56 -0. 86 ;0. 60 -0. 93）. MSM seek for AIDS service in CDC has higher HIV infection rate than those in hospital（ OR = 2.06,95% CI： 1.48 - 2. 88 ）. MSM with syphilis were more chance of HIV infection （ OR = 4. 68,95% CI：3/11 -7.04）. Conclusion HIV infection among MSM in Henan was higher. MSM aged under 20 years and beyond 40 years, ever - married or widowed, and with junior middle school education were high risk for HIV infection. Intervention for MSM should be targeted at MSM with high risk of HIV infection.
