血管内皮生长因子在肺癌中的临床应用价值

摘　　要：目的探讨血管内皮生长因子(VEGF)在肺癌中的临床应用价值。方法采用酶联免疫吸附试验(ELISA)检测142例经病理确诊的肺癌患者、30例肺部疾病对照患者及25名正常对照者血清VEGF水平,比较不同肿瘤分期、化疗和手术前后患者VEGF的表达改变,并用受试者工作特征(ROC)曲线评价VEGF在肺癌中的应用价值,采用SPSS 13.0统计软件进行统计分析。结果肺癌患者血清VEGF水平为154.09(86.81,260.01)pg/m L,明显高于正常对照组[59.06(27.66,77.81)pg/m L,P<0.01];不同病理类型肺癌患者间血清VEGF水平差异无统计学意义(P=0.493);患者化疗前后血清VEGF水平差异无统计学意义(P>0.05);不同M分期和临床分期的非小细胞肺癌患者VEGF水平差异有统计学意义(P<0.05);局限期与广泛期小细胞肺癌患者VEGF水平差异无统计学意义(P>0.05);VEGF诊断肺癌的ROC曲线下面积为0.664,区分小细胞肺癌远处转移的ROC曲线下面积为0.666。结论 VEGF在肺癌患者血清中异常表达,与肺癌患者临床分期密切相关,对肺癌的诊断及转移的判断具有一定价值。ObjectiveTo investigate the signiflcance of serum vascular endothelial growth factor（VEGF） in lung cancer. MethodsThe levels of serum VEGF were determined by enzyme-linked immunosorbent assay（ELISA） in 142 patients with lung cancer,30 patients with lung diseases and 25 healthy subjects. Meanwhile,their tumor stages and serum VEGF expression changes before and after chemotherapy and surgery were compared. The receiver operating characteristic（ROC） curve was used to evaluate the signiflcance of serum VEGF in lung cancer. SPSS 13.0 software was used for statistical analysis. ResultsSerum VEGF level in lung cancer group was 154.09（86.81,260.01） pg/mL,which was higher than that in healthy control group [59.06（27.66,77.81）pg/mL,P〈0.01]. Serum VEGF level had no statistical signiflcance for different pathological types of lung cancer group（P=0.493）. Serum VEGF levels before and after chemotherapy had no statistical signiflcance（P〉0.05）. Serum VEGF levels with different M stages and tumor stages in non-small cell lung cancer had statistical significance（P〈0.05）,and there was no statistical significance between localized and extensive stages（P〉0.05）. The area under ROC curve of serum VEGF in the diagnosis of lung cancer was 0.664,and that for non-small cell lung cancer distant metastasis was 0.666. ConclusionsThe expression of serum VEGF is abnormal in lung cancer group,and it is related to the clinical stages of lung cancer. It is of signiflcance in the diagnosis and metastasis of lung cancer.
