CPAP治疗难治性高血压合并OSAHS的临床研究

摘　　要：目的探讨无创气道正压通气(CPAP)治疗难治性高血压合并阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的临床效果。方法选取40例难治性高血压合并OSAHS患者,根据治疗方式的不同将其分为观察组21例和对照组19例,观察组在常规药物治疗基础上增加CPAP治疗,对照组患者单纯采取常规药物治疗,对比2组患者的治疗效果和觉醒指数变化。结果观察组治疗后醒后血压、夜间血压、AHI和Sa O2分别是(137±11)mm Hg、(81±5)mm Hg、(131±9)mm Hg、(80±4)mm Hg、(15.1±0.8)次/h、(94.5±2.5)%,对照治疗后醒后血压、夜间血压、AHI和Sa O2分别是(145±13)mm Hg、(93±7)mm Hg、(139±10)mm Hg、(90±7)mm Hg、(23.8±3.9)次/h、(88.4±3.1)%,组间比较差异均有统计学意义(P<0.05)。结论 CPAP治疗难治性高血压合并OSAHS的临床疗效确切,患者睡眠状况明显改善,值得在临床上推广使用。ObjectiveDiscussion of noninvasive positive airway pressure （CPAP） treatment of refractory hypertensive patients with obstructive sleep apnea hypopnea syndrome （OSAHS） clinical effect.Methods40 cases of patients with refractory hypertension combined OSAHS in this study, according to the different treatment modalities will be divided into 21 cases of observation group and the control group 19 cases, the observation group to increase CPAP treatment on the basis of conventional drug therapy, the control group of patients taking conventional drug therapy alone, compared to the therapeutic effect of the two groups of patients and arousal index change.ResultsObservation group after treatment, after waking SBP, DBP, nighttime SBP, DBP, AHI and SaO2 are （137 ± 11） mmHg, （81 ± 5） mmHg, （131 ± 9） mmHg, （80 ± 4） mmHg, （15.1 ± 0.8） times/h, （94.5±2.5）%, and waking SBP, DBP, nighttime SBP, DBP, AHI and SaO2 are （145 ± 13） mmHg, （93 ± 7） mmHg, （139 ± 10） mmHg, （90 ± 7） mmHg, （23.8 ± 3.9） times/h, （88.4±3.1）%, differences were statistically signiifcant between groups （P〈0.05）.ConclusionThe clinical efifcacy of CPAP treatment of refractory hypertension with OSAHS exact, sleep were signiifcantly improved and should be introduced in clinical practice.
