Wernicke脑病2例、原发性胼胝体变性2例报告及相关文献复习

摘　　要：目的探讨慢性酒精中毒、营养不良等因素致Wernicke’s encephalopathy和Marchiafava-Bignami disease的发病机制、临床表现、影像特点及治疗方案。方法报道4例典型病例,其中2例为Wernicke’s encephalopathy,2例为Marchiafava-Bignami disease,并总结复习了相关文献。结果结合4例典型病例和文献复习,总结分析了Wernicke’s encephalopathy和Marchiafava-Bignami disease的发病机制、临床表现、影像特点及治疗方案。结论慢性酒精中毒、营养不良等因素可通过多种机制导致Wernicke’s encephalopathy和Marchiafava-Bignami disease,临床上应予以重视。考虑到两病均为急症,早期诊断显得尤为重要。Objective To investigate the pathogenesis,clinical manifestations,radiologic features and treatment pro-tocol in patients with Wernicke’s encephalopathy or Marchiafava-Bignami disease secondary to chronic alcoholism,malnu-trition and so on.Methods Here we presented 4 typical cases,including the clinical manifestations,laboratory tests,treat-ment and outcome,among which two are Wernicke’s encephalopathy,the other two are Marchiafava-Bignami disease.Re-sults On the basis of case reports and literature review,the mechanism,clinical features,imaging characteristics and treat-ment strategy were analyzed.Conclusion Chronic alcoholism and malnutrition may be the two main causes for Wernicke’ s encephalopathy and Marchiafava-Bignami disease through complex mechanisms,to which should be paid enough attention on clinic.Taking into consideration of its emergency,it is of importance for early diagnosis.
