压力抗栓泵对重型颅脑损伤术后患者颅内压的影响

摘　　要：目的：探讨压力抗栓泵治疗对重型颅脑损伤（ STBI ）患者术后颅内压力（ ICP ）的影响。方法选取2013年4月—2014年9月在江苏省江阴市人民医院 NICU 接受治疗的 STBI 术后患者120例，随机分为研究组（60例）与对照组（60例），研究观察时间为术后6 d。研究组在术后前3 d接受压力抗栓泵治疗，第4-6天则停止该治疗，对照组则相反，持续监测所有患者ICP，并对数据进行统计学分析。结果两组患者ICP相关数据组内比较结果显示ICP均值在术后6 d均明显下降，ICP≥20 mmHg次数术后较术前均减少，每日ICP最高值术后逐渐下降，上述三项结局指标，前3 d与后3 d的差异均具有统计学意义（P＜0．05）。但两组患者每日ICP均值、ICP≥20 mmHg次数以及ICP最高值的比较结果显示，差异均无统计学意义（P＞0．05）。两组患者术后均未发生下肢DVT。结论 STBI患者术后应用压力抗栓泵可以有效预防DVT的发生，且不影响ICP，是一种安全有效的治疗措施。Objective To evaluate effect of antithrombotic pressure pump on the intracranial pressure （ICP） of the severe traumatic brain injury（STBI） patients who were performed the surgery.Methods A total of 120 consecutive STBI patients after the surgery , who were admitted at Neurological Intensive Care Units （NICU） from April 2013 to September 2014, were randomly divided into （research group and control group ）. Each group had 60 patients .In the 6 days observation , research group received the treatment of antithrombotic pressure pump among the first 3 days after the surgery , and the treatment was stopped among the last 3 days. Control group received the opposite treatment .ICP was continuously monitored and all the data （ mean ICP, times of ICP ≥20 mmHg, the highest ICP per day ） were performed statistical analysis .Results Mean ICP,times of ICP≥20 mmHg and the highest ICP per day in two groups decreased significantly after the surgery （ P＆lt;0.05）.The difference of Mean ICP, times of ICP ≥20 mmHg and the highest ICP per day between the two groups were not significant .No patient got the deep vein thrombosis （ DVT ） .Conclusions Antithrombotic pressure pump could efficiently prevent deep vein thrombosis , which is a safe treatment for STBI patients and has no influence on the ICP .
