摘　　要：目的:分析慢性不全梗阻性无功能肾的临床病理学和CD44表达情况,以便于将目前较多梗阻性肾病的研究结果应用于临床。方法:回顾性分析191例慢性不全梗阻性无功能肾的临床病理学特征,并用免疫组织化学法检测CD44的表达情况,并用Image-Pro Plus(IPP)6.0图像分析软件进行光密度检测。结果:慢性不全梗阻性无功能肾患者可有或无临床症状。镜下肾小球常萎缩消失,肾小管部分扩张,部分萎缩消失,间质炎细胞浸润,多少不等纤维组织及厚壁血管增生。CD44免疫组织化学分析显示慢性不全梗阻性无功能肾常呈强阳性,以肾间质炎性浸润区域明显。结论:慢性不全梗阻性无功能肾是临床常见肾切除病因。其病理表现为梗阻性肾病,但较实验性梗阻性肾病动物模型肾实质损害更加严重,且常伴化脓性炎、结核等病变。CD44表达增强与肾间质炎性浸润有关。Objective Analysis of clinical pathology and CD44 expression in chronic obstructive renal failure,so that the research of obstructive nephropathy results at present were applicated in clinica. Method Review of the clinical and pathological features of 191 cases of chronic obstruction of nonfunctioning kidneys,and the expression of CD44 were detected by immunohistochemistry method,and optical density was examined by IPP 6. 0 image analysis software. Results Chronic partial obstruction in patients without renal function were with or without clinical symptoms. The glomeruli often was atrophy and disappeared,tubulars were dilation and partial ones were atrophy,even disappeared,inflammatory cell infiltrated at interstitial parts,There were varying amounts of fibrous tissue and thick walled vessels. CD44 immunohistochemistry analysis showed that chronic obstruction nonfunctional kidney was often strongly positive,with renal interstitial inflammatory infiltration area. Conclusion Chronic obstruction without renal function is a common clinical cause of renal resection. The pathology showes obstructive nephropathy,but compared with the kidney model in experimental obstructive nephropathy animal the material damage is more serious,and often with purulent inflammation,tuberculosis and other diseases,The enhanced expression of CD44 is related with the inflammatory infiltration at renal interstitial.
