特应性皮炎患者高血清总IgE和胸腺活化调节趋化因子水平对其不良预后的影响分析

摘　　要：目的:探讨特应性皮炎(AD)患者高血清总免疫球蛋白E(Ig E)和胸腺活化调节趋化因子(TARC)水平对其不良预后的关系。方法:随机选取收治的AD患者78例作为AD组,另选取30例同期健康查体者作为对照。采用ELISA法检测两组的血清总Ig E和TARC水平。AD组均接受相应的治疗并定期行SCORAD评分评价并分析AD患者血清总Ig E和TARC水平与其SCORAD评分得分的关系。结果:与对照组比较,AD组治疗前后的血清总Ig E和TARC水平均升高;与治疗前比较,AD组治疗1周、2周、1个月和3个月的血清总Ig E和TARC水平均降低,差异有统计学意义(P<0.05)。AD组治疗前SCORAD评分亦均高于治疗1周、2周、1个月和3个月,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示,AD患者血清总Ig E和TARC水平与其SCORAD评分得分均呈正相关,差异有统计学意义(r=0.994,0.993,P<0.05)。结论:AD患者血清总Ig E和TARC水平较高且与其SCORAD评分得分相关,这可能与特应性皮炎引发患者免疫功能紊乱造成Ig E和TARC血清表达水平相关,血清总Ig E和TARC水平均与患者病情严重程度相关,两者高血清水平可能用于AD患者病情严重和不良预后评价的参考指标。Objective To explore the relationship between high serum total immunoglobulin E（ Ig E） and thymus and activation adjusting chemokine（ TARC） levels and poor prognosis of patients with atopic dermatitis（ AD）. Method 78 patients with AD were randomly selected as AD group,and the other 30 healthy persons had physical exam in our hospital in the same time,they were selected as control group. ELISA method was applied in detecting serum total Ig E and TARC level in both groups. AD group were treated with corresponding treatment and SCORAD scores were evaluated and relationship between serum total Ig E and TARC level and SCORAD scores of patients with AD were analyzed. Results Compared with control group,serum total Ig E and TARC level of AD group before and after treatment were higher; Compared with before treatment,serum total Ig E and TARC level of AD group after treated with 1 week,2 weeks,1 month and 3 months were lower,the difference was statistically significant（ P〈0. 05）. SCORAD scores of AD groups before treatment were also higher than that of after treated with 1 week,2 weeks,1 month and 3 months,the difference was statistically significant（ P〈0. 05）. Pearson correlation analysis results showed that serum total Ig E and TARC level in patients with AD were positively correlated with SCORAD scores（ r = 0. 994,0. 993,P〈0. 05）. Conclusion Serum total Ig E and TARC level in patients with AD are high and related with SCORAD scores. It may be related to atopic dermatitis of patients influencing immune function,causing associated disorders so as to increase Ig E and TARC serum expression level,thus serum total Ig E and TARC level are associated with disease severity in patients with high serum level and could be used in evaluation of both severe and poor prognosis in patients with AD.
