子宫剖宫产疤痕妊娠中宫腔镜的应用研究

摘　　要：目的分析研究子宫剖宫产疤痕妊娠中宫腔镜的临床应用价值。方法整群选取2012年2月—2015年2月在该院接收的剖宫产手术以后疤痕妊娠的患者一共有78例,采取数字随机法随机分为治疗组、对照组,每组39例。对照组对患者采取甲氨蝶呤药物进行保守治疗,治疗组对患者采取宫腔镜进行治疗,β-HCG下降一直到正常水平所需要的时间、住院时间、子宫疤痕妊娠终止率以及阴道大量出血率给予对比。对两组患者的临床治疗效果给予对比。结果临床治疗以后,治疗组子宫疤痕妊娠终率为92.31%,治疗组子宫疤痕妊娠终率为71.79%,治疗组患者的治疗成功率显著高于对照组,其之间的差异具有统计学意义(P<0.05);临床治疗以后,治疗组患者的住院时间为(7.81±4.02)d,β-HCG下降一直到正常水平所需要的时间为(95.31±2.65)d,对照组患者的住院时间为(14.97±6.26)d,β-HCG下降一直到正常水平所需要的时间为(10.42±3.18)d,治疗组患者的β-HCG下降一直到正常水平所需要的时间和患者住院时间明显低于对照组,其之间的差异具有统计学意义(P<0.05)。临床治疗以后,治疗组阴道大量出血率为7.7%,对照组阴道大量出血率为38.5%,治疗组患者的阴道大量出血率明显低于对照组,其之间的差异具有统计学意义(P<0.05)。结论对子宫剖宫产疤痕妊娠采取宫腔镜治疗,对患者病情恢复以及预后起到良好的促进作用。Objective To study the caesarean scar pregnancy hysteroscopy in clinical application. Methods Group selection in February 2012— February 2015 in our hospital received cesarean section scar pregnancy after surgery in patients with a total of 78 cases, adopt digital random method were randomly divided into treatment group and control group, control group patients take drug methotrexate for conservative treatment, the treatment group to take the hysteroscopic treatment of the patients, beta HCG fall until the time required to normal level, length of hospital stay, rate of uterine scar pregnancy termination and vaginal bleeding rate to give a large number of contrast. Results After clinical treatment, the treatment group of uterine scar pregnancy rate was 92.31%, the treatment group of uterine scar pregnancy rate was 71.79%, the treatment of patients with treatment group success rate is significantly higher than the control group, the differences statistically significant（P〈0.05）; After clinical treatment, the treatment group patients＇ length of hospital stay was（7.81±4.02）d, beta HCG drops until normal time was（95.31±2.65）d, the control of the patient＇s hospital stay was（14.97±6.26）d, beta HCG drops until normal time was（10.42±3.18）d, the treatment group patients of beta HCG down to normal levels need time and hospital stay in patients with significantly lower than the control group, the differences statistically significant（P〈0.05）. After clinical treatment, the treatment group vaginal bleeding badly at a rate of 7.7%, the control rate of 38.5%, vaginal bleeding badly vaginal bleeding badly rate of treatment group patients was obviously lower than the control group, the differences statistically significant（P〈0.05）.Conclusion To take the hysteroscopic treatment of the uterine cesarean section scar pregnancy,recovery and prognosis of the patient condition play a good role in promoting.
