电切术后曼月乐与妈富隆预防子宫内膜息肉复发的效果比较

摘　　要：目的探讨子宫内膜息肉电切术(TCRP)后放置曼月乐对预防子宫内膜息肉(EP)复发及减少月经量中的价值及意义。方法方便选取2010年1月—2013年1月于郑州市妇幼保健院就诊的子宫内膜息肉患者140例。按术后治疗方法分为3组,A组:术中放置曼月乐预防复发组(曼月乐组)45例;B组:术后口服妈富隆预防复发组(妈富隆组)45例;C组:仅行宫腔镜下息肉电切术(单纯电切组)50例。比较组间经量减少及EP复发的差异。结果治疗后,相同随访时段内,月经量由少到多依次为:A组(曼月乐组)<B组(妈富隆组)<C组(单纯电切组)(P<0.05);子EP复发情况比较,由低到高依次:曼月乐组2.2%(1/45)<妈富隆组6.6%(3/45)<单纯电切组8%(4/50)。以上组间差异均具有统计学意义(P<0.05)。结论使用曼月乐月经量的减少程度较妈富隆明显,息肉复发率较妈富隆低。且对于患者使用方便,依从性高。值得临床广泛推广。Objective To study the difference of Levonorgestrel intrauterine System and Marvelon for recurrence of endometrial polyps and menstrual flow. Methods Convenient selection of 140 cases of endometrial polyps in January 2013 — January 2010 in Zhengzhou maternal and child health hospital. According to the postoperative treatment were divided into three groups, group A： intraoperative Mirena relapse prevention group（IUS group） 45 cases; group B： postoperative Marvelon oral prevention recurrence group（Marvelon group） 45 cases; group C： only hysteroscopic polyp electrical cutting operation（simple cutting group） 50 cases. Compare the difference between the reduction of the amount of the group and the recurrence of EP. Results After treatment, the same follow-up period, menstrual quantity from less to more, followed by： Mirena group（group A）〈 group B（Marvelon）〈 C group（simple resection group（P〈0.05）; sub EP recurrence compared from low to high as follows： Mirena group 2.2%（1/45）〈 Marvelon group 6.6%（3/45）〈 pure electric cutting is 8%（4 / 50）. The differences between the above groups were statistically significant（P〈0.05）. Conclusion After TCRP,the Levonorgestrel intrauterine System can effectively prevent EP recurrence.
