巴曲酶联合长春西汀对老年突发性耳聋的临床疗效研究

摘　　要：目的研究巴曲酶联合长春西汀对老年突发性耳聋的临床疗效。方法选择2013年1月～2015年12月在我院进行诊治的老年突发性耳聋患者176例，随机分为两组，长春西汀组给予长春西汀，联合用药组给予巴曲酶联合长春西汀，观察两组的治疗效果、血液流变学指标、凝血功能和纯音听阈值。结果联合用药组的有效率为89．78％，明显高于长春西汀组的70．54％（P〈0．05）；两组治疗后的全血黏度低切、全血黏度高切、血浆黏度和血细胞比容均明显降低（P〈0．05），且联合用药组降低的更为明显（P〈0．05）；两组治疗后的活化部分凝血活酶时间、凝血酶时间、凝血酶原时间均明显升高（P〈0．05），血浆纤维蛋白原明显降低（P〈0．05），两组相比无明显差异（P〉0．05）；两组治疗后的纯音听阈值均明显改善（p〈0．05），且联合用药组更为明显（P〈0．05）。结论巴曲酶联合长春西汀对老年突发性耳聋治疗效果显著，可以有效改善患者血液高凝状态和血液流变学状态。Objective To investigate the clinical effects of batroxobin combined with vinpocetine in the treatment of senile sudden deafness. Methods 176 cases of senile patients with sudden deafness were selected and treated at our hospital from January 2013 to December 2015. They were divided into two groups randomly.The vinpocetine group was given vinpocetine, the combination group was given batroxobin combined with vinpocetine. The therapeutic effects, blood rheology, coagulation functions and pure tone thresholds of the two groups were observed. Results The effective rate of combination group was 89.78%（79/88）, significantly higher than that of the vinpoeetine group which was 70.54% （62/88） （P〈0.05）. After treatment, low shear whole blood viscosity,high shear whole blood viscosity,plasma viscosity and hematocrit of the two groups were significantly reduced（P〈0.05） and the combination group decreased more obviously （P〈0.05）. The activated partial thromboplastin time, and thrombin time,prothrombin time were significantly increased （P〈0.05）, while plasma fibrinogen decreased significantly （P〈0.05）. The two groups had no significant differences （P〉0.05）. The pure tone thresholds of the two groups were significantly improved （P〈0.05）, and the combination group was more obvious （P〈0.05）. Conclusion Batroxobin combined with vinpocetine had noticeable effects on the treatment of senile sudden deafness, and could effectively improve the patient＇s blood hypercoagulable state and hemorheology state.
