兔自体骨膜包裹肌腱移植前交叉韧带重建实验研究  

Experimental Study on Anterior Cruciate Ligament Reconstruction With Autologous Periosteum Wrapped Tendon in Rabbits

摘　　要：目的观察比较翻转骨膜包裹自体肌腱移植、单纯自体肌腱及骨膜正常包裹自体肌腱移植,重建前交叉韧带移植物腱骨愈合的组织学变化过程。方法取60只新西兰白兔120膝,随机平均分为翻转骨膜组、正常骨膜组及肌腱组。切除兔双膝前交叉韧带,取其双侧拇长伸肌腱,及需要切取骨膜侧胫骨上端内侧骨膜,大小约1.5 cm×0.5 cm。同侧肌腱折成2股重建兔双膝前交叉韧带。采用HE染色、甲苯胺蓝染色,观察重建术后3、4、8及12周时移植物腱骨愈合的变化情况。结果显微镜下观察重建术后随着时间推移,翻转骨膜组较其他两组腱骨界面可见成纤维细胞、纤维软骨细胞更多表达;12周时,翻转骨膜组腱骨界面间的胶原纤维数量及排列较其他两组无明显差异;生物力学测试术后3、4、8周骨膜翻转组抗牵拉力明显大于其他两组,而术后12周组间对照无统计学差异。结论骨膜包裹自体肌腱移植重建兔前交叉韧能够在愈合期12周内促进腱骨结合部成纤维细胞、骨细胞及软骨细胞的再生;对腱骨愈合具有积极的促进作用,超过愈合期则无差异。Objective To compare reverse periosteum wrapped autologous tendon transplantation, autologous tendon and normal periosteum wrapped with autologous tendon transplantation, reconstruction of anterior cruciate ligament graft tendon bone healing histological change. Methods 60 New Zealand white rabbits 120 knees were selected, the reversed periosteum group, the normal periosteum group and the tendon group. Resection of rabbit knee anterior cruciate ligament, the bilateral extensor pollieis longus tendon, and the need to cut the proximal tibia medial periosteal periosteum, the size of about 1.5 cmx0.5 cm. The same side of the tendon folded into 2 strands of anterior cruciate ligament reconstruction. Using HE staining, toluidine blue staining, observe the reconstruction after 3, 4, 8 and 12 weeks of shift plant tendon bone healing. Results Observation under the microscope after reconstruction with time reversal periosteum group than the other two groupsof tendon bone interface fibroblasts, cartilage cells more expression, at 12 weeks, flip periosteum group tendon bone interface between the number and arrangement of the collagen fibers than the other two groups had no significant difference; biomechanical test after 3 and 4, 8 week group of anti pulling force of periosteal turnover was significantly greater than the other two groups, and 12 weeks after operation there was no significant difference between groups. Conclusion Reconstruction of periosteum wrapped tendon autograft of rabbit anterior cruciate ligament can promote the regeneration of tendon bone healing in 12 weeks with fibroblasts, osteoblasts and chondrocytes; which plays a positive role in promoting the healing of tendon bone healing period, there were no differences over.
