心房颤动患者血浆丙二醛及谷胱甘肽过氧化物酶的表达

摘  要:目的研究心房颤动(房颤)患者血浆氧化应激指标及其与心房结构重构的关系。方法选取在徐州医科大学附属医院行心脏外科手术的心脏瓣膜病患者43例,按照是否合并房颤分成2组:持续性房颤组(AF组)21例,其房颤持续时间超过7天;窦性心律组(SR组)22例。在体外循环手术开始前收集外周静脉血,检测丙二醛(MDA)及谷胱甘肽过氧化物酶(GSH—Px)的含量。术前完善心脏彩超检查,记录左心房内径(LAD)。结果房颤组患者LAD为(56.90±9.93)mm,较窦性心律组[(40.59±9.40)mm]明显增高(P〈0.01)。房颤组和窦性心律组MDA含量分别为(8.07±0.32)、(4.23±0.23)nmol/mg,房颤组明显高于窦性心律组(P〈0.05);房颤组和窦性心律组GSH—Px表达分别为(127.26±12.12)、(168.91±18.25)U,房颤组明显低于窦性心律组(P〈0.05)。房颤患者MDA与LAD明显正相关(P〈0.05)。结论心脏瓣膜病合并房颤的患者氧化应激增强,氧化应激可能参与心房结构重构及房颤的发生。Objective To analyze the levels of plasma oxidative stress biomarkers in atrial fibrillation (AF) and their relationship with atrial structural remodeling. Methods A total of 43 patients with valvular heart disease in the Affiliated hospital of Xuzhou Medical University were divided into two groups : a continuous AF group ( Group AF, where the fibrillation lasted more than 7 days, n = 21 ) and a sinus rhythm group (Group SR, n = 22). The peripheral venous blood samples were collected before surgery to determine the amounts of malondialdehyde (MDA) and glutathione peroxidase ( GSH - Px). Ultrasonic cardiogram reports were collected before surgery to analyze the left atrial diameter (LAD). Results The LAD was (56.90 ± 9.93) mm for Group AF, which was significantly larger than that in Group SR (40.59 ±9.40) mm (P 〈 0. 01 ). The content of MDA was (8.07 ±0. 32) nmol/mg in Group AF, which was significantly larger than that in Group SR (4.23 ± 0. 23 ) nmol/mg ( P 〈 0.05 ). The content of GSH - Px was ( 127.26 ± 12.12) g in Group AF, which was remarkably lower than that in Group SR (168.91 ± 18.25) U (P 〈0.05). MDA was markedly related with LAD in AF patients (P 〈 0. 05 ). Conclusions Oxidative stress is significantly enhanced in AF patients with valvular heart disease. Oxidative stress is likely to be involved in structural remodeling and atrial fibrillation.
