超声多普勒引导下痔动脉结扎术联合连续缝合悬吊术治疗脱垂性痔的临床观察

摘　　要：目的观察超声多普勒引导下痔动脉结扎术(DG-HAL)联合连续缝合悬吊术治疗脱垂性痔的临床疗效。方法采用随机对照试验的方法,将40例脱垂性痔患者随机分为治疗组与对照组,每组各20例。治疗组行DG-HAL联合连续缝合悬吊术,对照组行Milligan-Morgan术。比较2组临床疗效、住院时间、创面愈合时间及术后并发症的差异。结果 2组临床疗效、排便困难及肛门坠胀感比较差异均无统计学意义(P均>0.05),而治疗组住院时间、创面愈合时间均明显短于对照组(P均<0.05),术后肛门疼痛、术后出血、肛缘水肿、排尿困难均明显轻于对照组(P均<0.05)。结论 DG-HAL联合连续缝合悬吊术治疗脱垂性痔具有住院时间短、有效、微创、术后疼痛及水肿轻等优点,从而提高患者满意度,是治疗脱垂性痔的较好术式。Objective It is to observe the clinical efficacy of Doppler-guided hemorrhoid artery ligation with running suture and suspension on prolapsed hemorrhoids. Methods Forty cases of prolapse and hemorrhoids patients were randomly divided into two groups： a treatment group and control group by random-control test,each group had 20 cases. The treatment group were treated by Doppler-guided hemorrhoid artery ligation with running suture and suspension,and the control group were treated by Milligan-Morgan hemorrhoidectomy. The clinical curative effect,hospital stay,wound healing time,postoperative complications in each group were compared. Results No significant difference was found in the clinical curative effect,defecation difficulties,anal drop bulge between the two groups（ P 0. 05）. The hospital stay,wound healing time,postoperative pain,postoperative bleeding volume,swelling and urination difficulties in the treatment group were significantly better than those in the control group（ P 0. 05）. Conclusion Doppler-guided hemorrhoid artery ligation with running suture and suspension presents shorter hospitalization time,effective,minimally invasive,alleviate postoperative pain and swelling on prolapsed hemorrhoids compared with Milligan-Morgan hemorrhoidectomy,so it wins the satisfaction of the patients and be worthy of widely used.
