彩色多普勒超声闪烁伪像在超声定位的体外冲击波碎石中的价值

摘　　要：目的探讨彩色多普勒超声闪烁伪像在以超声定位的体外冲击波碎石中的应用价值。方法选择我院2013年1月-2015年12月收治的316例输尿管结石患者，所有病例普通B超均未发现结石，后经其他影像学检查确诊输尿管结石，再行彩色多普勒超声检查，结合其他影像学检查提示结石解剖位置，于可疑结石强回声处，运用彩色多普勒超声闪烁伪像技术，重点扫查，观察结石的检出率变化。结果本组316例输尿管结石患者，运用彩色多普勒超声闪烁伪像技术发现输尿管结石，最后体外冲击波碎石可定位的为248例，总定位成功率78％，其中A组定位成功率81％，B组63％，C组85％，A组与B组比较差异有统计学意义（P〈0．05），B组与C组比较差异有统计学意义（P〈0．05），A组与C组比较无统计学意义（P〉0．05）。结论彩色多普勒超声闪烁伪像可有效提高体外冲击波碎石（ESWL）的定位成功率，但对于B组患者，其定位成功率仍相对较低。Objective To investigate the application value of color Doppler tinkling artifact in the ultrasonic localized extracorporeal shock wave lithotripsy. Methods 316 patients with ureteral calculus from January 2013 to December 2015 were selected and no stones were foundby ordinary Ultrasound B in all patients, All the patients were diagnosed ureteral calculus by other imaging examinations and color Doppler ultrasound was performed then. The anatomical positions of the stones were considered to be at the points with strong echo combined with other imaging examination. The color Doppler twinkling artifact was applied for the scan and examination and the detective rate of the stone was observed. Results For the 316 patients with ureteral calculus in this study, the ureteral stone was detected by the color Doppler twinkling artifact and the final localized extracorporeal shock wave lithotripsy were 248 cases with the total successful rate of localization of 78%. The successful rate of localization of group A was 81%, and that of group B was 63% and that of group C was 85%. Statistical significance was presented in the differences between group A and B （P〈0.05）, as well as between group B and C （P〈0.05）, while no statistical significance was found in the differences between group A and C （P〉0.05）. Conclusion Color Doppler twinkling artifact can effectively increase the successful rate of localization of ESWL, however, the successful rate of patients from group B is still relatively low.
