摘  要:①目的探讨胆管结石和胆囊结石患者胆汁酸(TBA)、胆红素(STB)及胆固醇(TC)等血液中代谢变化特点及与健康人群的关系。②方法收集临床确诊的胆管结石患者23例,男16例,女7例,平均年龄(43±6.24)岁;胆囊结石组31例,男19例,女12例,平均年龄(45±3.16)岁;对照组为健康体检人员39例,男26例,女13例,平均年龄(44±8.52)岁。分别于清晨空腹静脉采血,测定血液中TBA、TC、STB、结合胆红素(CB)和未结合胆红素(UCB)浓度。③结果胆管结石组和胆囊结石组TBA显著高于对照组(P<0.01);胆管结石组TBA显著高于胆囊结石组(P<0.01)。胆管结石组和胆囊结石组TC均高于对照组(P<0.05);胆管结石组TB高于对照组(P<0.05)。④结论胆结石的形成与胆汁酸代谢密切相关,胆管结石的形成和发展又促进血液胆汁酸浓度的进一步升高,当血液胆汁酸达到一定浓度后也会引起血液胆红素的升高。Objective To explore the metabolic change characteristics of bile acids and bilirubin in patients with bile duct and gallbladder,and the relationship with healthy people.Methods A total of 23 patients in biliary calculi,16 males and 7 females,the average age was(43±6.24).31 patients in gall-bladder stones,19 males and 12 females,the average age was (45±3.16).39 cases of healthy volunteers as control group,26 males and 13 females,the average age was (44±8.52).Took fasting venous blood in the morning,to test the total bile acid (TBA),total cholesterol (TC),serum total bilirubin (STB), conj ugated bilirubin (CB)and unconj ugated bilirubin (UCB)using enzymatic method.Results TBA in biliary calculi and gallbladder stones groups was significantly higher (P 〈0.01)than that in control group,TBA in biliary calculi group was significantly higher (P〈0.01)than that in gallbladder stones groups,and TC in biliary calculi and allbladder stones groups was higher(P 〈0.05)than that in con-trol group,STB in biliary calculi group was higher (P 〈0.05)than that in control group.Conclusion The formation of gallstones is related to bile acid metabolism,the formation and development of biliary calculi yet promote further bile acid rise further,when blood bile acid concentration reaches a certain level,bilirubin concentration will also increase.
