摘 要:目的探讨快吸慢呼训练对慢性心力衰竭患者运动耐力的影响。方法选取2014年12月至2015年10月入住我院心内科病房的慢性心力衰竭患者,按照病区进行分组,将十五(一)病区的42例患者作为实验组,十五(二)病区的39例患者作为对照组。两组均接受心内科常规治疗及护理,实验组进行快吸慢呼训练,于住院期间、干预1个月、3个月测量患者6 min步行距离、评估肺功能、静息心率(入院第2天)并计算平均静息心率(干预1个月、3个月)。结果重复测量方差分析结果显示,两组6 min步行距离、静息心率、用力肺活量、第一秒用力呼气量、最大自主分钟通气量的组间效应及时间效应差异有统计学意义(P<0.05),其中6 min步行距离、静息心率、用力肺活量、最大自主分钟通气量的干预与时间因素存在交互效应(P<0.05)。结论快吸慢呼训练可提高慢性心力衰竭患者的运动耐力。Objective To evaluate the effects of inspiratory muscle training by fast inhaling and slow exhaling on exercise endurance in patients with chronic heart failure. Methods From December 2014 to October 2015,a total of 81 patients with chronic heart failure were recruited and randomly divided into the experimental group (42 cases) and the control group(39 cases) according to different wards. The No.1 of fifth ward was as the experimental group, and the No. 2 of fifth ward was as the control group. All patients received routine treatment and nursing care. Additionally,the experimental group received inspiratory muscle training. Six-minute walk distance,pulmonary function,resting heart rate(2nd day in hospital) and average resting heart rate(1 month and 3 months after intervention) were measured during hospitalization, 1 month and 3 months after intervention. Results Repeated Measures Analysis of Variance showed that the differences between the two groups in the main effects of intervention and time on six-minute walk distance(P〈0.05),resting heart rate(P〈0.05),forced vital capacity(P〈0.05),forced expiratory volume in one second(P〈0.05) and maximal voluntary ventilation(P〈0.05) were all statistically significant. The results also demonstrated that six-minute walk distance,resting heart rate,forced vital capacity,maximal voluntary ventilation was associated with the time factor,indicating that they interacted with each other(P〈0.05). Conclusion Inspiratory mus- cle training by fast inhaling and slow exhaling can enhance the exercise tolerance of patients with chronic heart failure.
