维持性血液透析患者透析相关指标的城乡差异

摘　　要：目的观察维持性血液透析(MHD)患者透析相关指标的城乡差异。方法比较南京市江宁区城市MHD患者(A组,124例)和农村MHD患者(B组,14例)血液透析前后透析相关指标。结果 B组血液透析前血肌酐和血尿素氮均高于A组[(810.4±101.2)μmol/L vs.(718.9±151.7)μmol/L和(33.3±3.9)mmol/L vs.(30.1±5.2)mmol/L](P<0.05)。血液透析后,B组透析后Kt/V值低于A组(1.0±0.4vs.1.3±0.5),而血肌酐、血尿素氮和血磷水平均高于A组[(313.2±78.8)μmol/L vs.(273.2±70.9)μmol/L、(11.1±1.6)mmol/L vs.(9.9±2.3)mmol/L和(2.1±0.5)mmol/L vs.(1.5±0.5)mmol/L](P<0.05或P<0.01)。结论农村MHD患者的透析充分性较城市MHD患者差。Objective To investigate the differences in dialysis-related indicators in maintenance hemodialysis（MHD） patients living in city and country. Methods Dialysis-related indicators of MHD patients from city（group A, 124 cases） and country（group B, 14 cases） in Jiangning district of Nanjing were compared. Results Serum levels of creatinine and urea nitrogen were higher in group B than those in group A before hemodialysis[（810. 4±101.2）μmol/L vs. （718.9±151.7）μmol/L and（33.3 ±3.9） mmol/L vs. （30.1 ±5.2） mmol/L] （P〈0.05）. After hemodialysis, the value of Kt/V was lower（1.0±0.4 vs. 1.3 ± 0.5）, but serum levels of creatinine, urea nitrogen and phosphorus were higher,in group B than those in group A[（313.2±78.8） μmol/L vs. （273.2±70. 9）μmol/L, （11. 1± 1.6） mmol/L vs. （9.9±2.3） mmol/L,and （2.1±0.5） mmol/L vs. （1.5±0.5） mmol/L] （P〈0.05 or P〈0. 01）. Conclusion The adequacy of MHD is less in the patients living in rural area than that in those living in city.
