摘 要:目的 分析血清抗苗勒管激素对体外受精-胚胎移植(IVF-ET)妊娠结局的影响。方法 随机选取185例进行IVF-ET治疗的患者,于月经第2-4天测定血清促卵泡刺激素(FSH)、血清促黄体生成素(LH)和血清雌二醇(E2)水平,同时用超声行卵巢窦卵泡计数(AFC);在超促排卵治疗前测定血清抗苗勒管激素(AMH)水平;根据新鲜周期和冷冻胚胎复苏周期移植妊娠结局分组比较各观察指标。结果 1血清AMH与获卵数、成熟卵数、受精卵数、可移植胚胎数和优质胚胎数均有显著相关性(P〈0.001)。2血清AMH与临床妊娠率无统计学差异(P〉0.05),但在新鲜周期移植中,血清AMH与临床妊娠率呈正相关(P〈0.05),有统计学意义。3血清AMH不能预测IVF-ET的妊娠结局(ROC-AUC=0.483)。结论 1血清AMH与获卵数、成熟卵数、受精卵数、可移植胚胎数和优质胚胎数有显著相关性。2血清AMH不能预测IVF-ET的妊娠结局,在新鲜周期移植中,血清AMH与年龄较小患者的临床妊娠率有相关性。Objective To analyze the effect of serum anti-Mullerian hormone (AMH) on pregnancy outcome of in vitro fertilization- embryo transfer (IVF-ET) . Methods A total of 185 patients treated by IVF-ET were randomly selected. The levels of serum follicle- stimulating hormone ( FSH), luteinizing hormone ( LH), and estradiol ( E2 ) were detected from the second day to the fourth day of menstru- al cycle. Ultrasound was used to conduct antral follicle count (AFC) . Before controlled ovarian hyperstimulation ( COH), the levels of ser- um AMH were detected. The patients were grouped according to pregnancy outcomes of fresh cycle transfer and frozen-thawed embryo trans- fer, the observation indexes were compared. Results Serum AMH was significantly correlated with the numbers of retrieved oocytes, mature oocytes, fertilized oocytes, transplantable embryos, and good-quality embryos (P〈0. 001 ) . There was no significant correlation between serum AMH and clinical pregnancy rate (P〉0. 05) , but in fresh cycle transfer, serum AMH was positively correlated with clinical pregnancy rate (P〈0. 05) . Serum AMH could not predict pregnancy outcome of IVF-ET ( ROC-AUC = 0. 483) . Conclusion Serum AMH is sig- nificantly correlated with the numbers of retrieved oocytes, mature oocytes, fertilized oocytes, transplantable embryos, and good-quality em- bryos. Serum AMH can not predict pregnancy outcome of IVF-ET. In fresh cycle transfer, serum AMH is correlated with clinical pregnancy rate of young pregnant women.
