临床护理路径在腰椎间盘突出症手术患者中应用的Meta分析

摘　　要：目的采用Meta分析方法系统评估临床护理路径在腰椎间盘突出症手术患者中的应用效果。方法计算机检索中国学术期刊全文数据库（CNKI）、中文科技期刊全文数据库（VIP）、万方数据库、重庆维普数据库等中文数据库，检索关于临床护理路径在腰椎间盘突出症手术患者中应用效果的相关研究，按照纳入与排除标准筛选文献、提取资料并评价纳入研究的质量后采用RevMan5．3软件进行Meta分析，计算合并OR值及其95％CI，并进行发表偏倚评估及敏感性分析。结果共47项研究纳入，包括4774例腰椎间盘突出症患者。Meta分析结果表明，临床护理路径较常规护理组显著减少患者的住院时间、住院费用和并发症发生率[SMD：-1．85，95％C1（-2．16，-1．54），P〈0．01]，[OR=0．23，95％CI（0．18，0．31），P〈0．01]，[SMD=-2．72，95％CI（-3．33，-2：12），P〈0．01]，并能提高患者满意度、健康知识知晓率和功能锻炼达标率[OR=6．16，95％CI（4．59，8．25），P〈0．01]，[OR=5．88，95％CI（4．45，7．77），P〈0．01]，[OR=7．40，95％a（4．62，1I．85），P〈0．01]。结论临床护理路径在腰椎间盘突出症手术患者中应用效果显著优于传统护理方法。Objective To evaluate the application effect of clinical nursing path in surgery patients with lumbar disc prolapse in Meta analysis of published literature. Methods We searched CNKI, VIP, Wanfang database, CQVIP database in Chinese for the research related to application effects of clinical nursing path in surgery patients with lumbar disc prolapse. According to include and exclude criteria screening the literature, the data was extracted and evaluated the quality then had been done Meta analysis with RevMan 5.3 software, calculated OR combined with 95% CI, assessed publication bias and analyzed sensitivity. Results A total of 47 study were included involved 4 774 patients with lumbar disc prolapse. The results of Meta analysis showed that elinieal nursing path could reduce the length of hospital stay of patients, hospitalization expenses and complication rates [SMD = - 1.85, 95% CI （ - 2. 16, - 1.54）, P 〈 0. 01 ], [ OR = 0. 23, 95% CI（O. 18, 0.31）, P〈0. 01], [SMD = -2. 72, 95%CI （ -3. 33, -2. 12）,P 〈0. 01],and improve patient＇s satisfaction, awareness of knowledge about health and the success rate of functional exercise [ OR =6. 16, 95% C1（4.59,8.25）, P〈0.01], [0R=5.88, 95%CI（4.45,7.77）, P〈0.01], [0R=7.40, 95%CI（4.62, 11.85 ）, P 〈 0.01 ]. Conclusions Application effect of clinical nursing path in surgery patients with lumbar disc prolapse is significantly superior to that of traditional, nursing method.
