专项整治后福建医科大学附属闽东医院抗菌药物使用趋势调查与分析

摘　　要：目的:了解抗菌药物临床应用专项整治后福建医科大学附属闽东医院(以下简称"我院")抗菌药物使用情况及其合理性,为继续深入开展抗菌药物临床应用专项整治工作提供参考。方法:从医院信息系统中提取2011—2014年我院抗菌药物使用数据,对抗菌药物使用强度(antibiotics use density,AUD)、抗菌药物使用率(使用人数)和用药频度(defined daily dose system,DDDs)等进行统计分析。结果:专项整治活动期间,抗菌药物从2011年的127个品规降为2014年的68个品规;住院患者抗菌药物使用率从2011年的57.06%降为2013年的47.15%,AUD从2011年的46.97 DDDs/(100人·d)降为2013年的32.60 DDDs/(100人·d);但是,2014年抗菌药物使用率为53.51%、AUD为35.68 DDDs/(100人·d),又有上升趋势。结论:3年的抗菌药物临床应用专项整治工作效果显著,但还存在不合理用药现象。因此,医院应重视抗菌药物的合理应用,制定可持续、循环的改进制度和措施。To investigate the application status and rationality of antibiotics in inpatients from Mindong Hospital affiliated to Fujian Medical University after special rectification （ hereinafter referred to as ＂our hospital＂ ） , so as to provide reference for the further development of special rectification in clinical antibiotics. METHODS ： The application data of antibiotics during 2011-2014 were extracted from the hospital information system（ HIS）, statistical analysis was conducted on the antibiotics use density （AUD）, application rate of antibiotics （number of cases ） and defined daily dose system（DDDs） , etc.. RESULTS： During special rectifications, the product regulations of antibiotics decreased from 127 to 68 in 2014; the application rate of antibiotics in inpatients decreased from 57.06% in 2011 to 47.15% in 2013; and AUD decreased from 46.97 DDDs/（ 100 persons·d）in 2011 to 32.60 DDDs/（ 100 persons.d）in 2013. However, the application rate of antibiotics was 53.51% in 2014, and AUD was 35.68 DDDs/（ 100 persons·d） , both of which were in a increasing trend. CONCLUSIONS ： The effects of three-year special rectification were significant, yet there are still some irrational phenomenon. It is necessary application of antibiotics, and formulate sustainable, periodic for the hospital to attach more importance to the rational improvement regulations and measures.
