873例住院患者经饲管给药的合理性分析

摘　　要：目的:了解中山大学附属第一医院(以下简称"我院")住院患者经饲管给药的药物使用情况并探讨其合理性,为临床合理经饲管给药提供参考。方法:采用回顾性调查方法,对我院2014年1月1日—12月31日873例成人住院患者经管饲给药情况及其合理性进行评价分析。结果:纳入的873例住院患者经饲管给药共101 693例次,涉及249种药物。其中药物品种选择不适宜共涉及34种药物,合计使用6 667例次,不合理用药率为6.56%。不合理用药的主要问题包括将舌下含片及缓、控释制剂研碎后经饲管给药;将肠溶制剂研碎后通过胃管给药;软胶囊定量不准确;高渗溶液不经稀释直接经饲管给药;磨碎致癌、致畸、细胞毒性药物等。另外,还需关注药物与肠内营养液之间的相互作用。结论:临床经饲管给药存在不合理应用情况,临床药师应利用药物专业知识进行适当干预、监护,并与医护人员共同制订相应规范并实施,保证药物治疗的安全、有效。OBJECTIVE： To investigate and explore the application and rationality of medication through feeding tubes for inpatients in the First Affiliated Hospital of Sun Yat-sen University（ hereinafter referred to as ＂our hospital＂）,so as to provide reference for the rational medication through feeding tubes in clinic. METHODS： Retrospective investigation method was adopted,the application and rationality of medication through feeding tubes for 873 inpatients from Jan. 1st to Dec. 31st2014 were evaluated and analyzed. RESULTS： A total of 873 inpatients were included,administrated 249 kinds of drugs via feeding tubes for 101 693 case-times aggregately. 34 kinds of drugs were inappropriate selection,totally 6 667 case-times,accounting for 6. 56%. The main issues were as follows： medication through feeding tubes by crushing the Sublingual tablets and modified-release formulations,medication through stomach tubes by crushing enteric-coated preparation,inaccurate quantity of soft capsule,hypertonic liquid directly into medication through feeding tubes without diluting,crushing carcinogenic,teratogenic and cytotoxic properties,etc.. In addition,it is necessary to attach importance on the interactions between drugs and enteral nutrition fluids. CONCLUSIONS： There are still irrational phenomenon in the application of medication through feeding tubes in clinic. The clinical pharmacists should intervene and monitor the irrational phenomenon by the professional knowledge,and formulate corresponding specifications with the medical staffs,so as to ensure the safe and effective medication for patients.
