置双腔管主动引流联合VSD负压引流治高位肛周脓肿的初步研究

摘　　要：目的研究置双腔管主动引流联合VSD负压引流治疗高位肛周脓肿的效果。方法选择2014年4月到2015年8月到我院进行治疗的100名高位肛周脓肿患者作为研究对象,随机分为两组,对照组患者50名、选用传统切开高位挂线技术,观察组患者50名,选用置双腔管主动引流联合VSD负压引流技术。观察两组患者伤口长度、换药疼痛积分、愈合时间、失禁评分、治疗后复发高位肛周脓肿、肛瘘几率以及六月后治疗效果比较情况。结果经不同处置后,观察组伤口长度为(3.14±0.84)cm、换药疼痛积分为(16.38±3.42)分、愈合时间为(21.20±5.20)d、失禁评分为(0.05±0.23)分,均明显小于对照组,差异有统计学意义(P<0.05)。治疗结束后发现观察组仅有3例再次复发肛周脓肿、肛瘘,对照组达21例,差异有统计学意义(P<0.05)。观察组住院时间及住院费用,分别为(15.32±4.10)d及(3508.26±572.62)元,远少于对照组,差异有统计学意义(P<0.05)。在治疗结束六月后,随访发现观察组治疗有效达49例,仅1例无效,相比对照组,有效率明显提高,差异有统计学意义(P<0.05)。结论治疗高位肛周脓肿,采用置双腔管主动引流联合VSD负压引流,相比传统切开高位挂线技术,可以明显缩小伤口,减少换药时的痛苦及伤口愈合时间,而且显著减低术后复发情况,同时减少患者住院费用和住院时间,很大程度上提高了治疗质量。Objective To study A preliminary study of vacuum-assisted treatment of high perianal abscess by setting double lumen tube joint vacuum sealing drainage. Methods There were 100 patients with high perianal abscess who came to our hospital for treatment from April 2014 to August 2015 as research subjects. By the random number table method, patients were divided into two groups, 50 patients with high thread traditional incision technique; observation group were setting using double-lumen tube drainage combined with VSD. Observe the wound length, dressing pain score, healing time, incontinence score, relapse of high perianal abscess, anal fistula treatment and treatment effect after six months. Results After different disposition, we found the wound length of the observation group was（3.14 ± 0.84） cm, dressing pain score was（16.38 ± 3.42） points, the healing time was（21.20 ± 5.20） days, incontinence score was（0.05 ± 0.23） points, were significantly less than the control group, the difference was statistically significant（P〈0.05）. After treatment, the observation group found that only 3 cases of relapse perianal abscess, anal fistula, while the control group was as high as 21 cases, significant differences were statistically significant（P〈0.05）. And after discharge, collect statistics of each group hospitalization and hospital costs, and the observation group were（15.32 ± 4.10） days and（3508.26 ± 572.62） yuan,much lower than the control group, the difference was statistically significant（P〈0.05）. At the end of six months of treatment, followed up the observation group found that effective treatment is up to 49 cases, only 1 case is invalid, and compared to the control group, the efficiency increases, the difference was statistically significant（P〈0.05）.
