下髎穴注射治疗肛提肌综合征40例疗效观察

摘　　要：[目的]探讨下髎穴注射治疗肛提肌综合征的疗效。[方法]将80例患者随机分为治疗组(n=40)和对照组(n=40),治疗组采用下髎穴注射治疗,对照组采用生物反馈疗法治疗,对治疗前后疼痛评分、生活质量评分、肛管测压进行评估,并比较临床疗效与随访6月复发率。[结果]两组患者在疼痛评分、肛管静息压、肛管收缩压等方面,治疗后较治疗前均有不同程度改善,治疗组改善程度优于对照组,差异有统计学意义(P<0.05);在生理机能(PF)、躯体疼痛(BP)和社会功能(SF)等生活质量评定指标方面,治疗组改善程度优于对照组,差异有统计学意义(PF、SF比较,P<0.05;BP比较,P<0.01);治疗组临床有效率为87.50%,对照组为75.00%,两组比较差异有统计学意义(P<0.05);随访6月,治疗组与对照组复发率分别为15.00%、27.50%,两组比较差异无统计学意义。[结论]下髎穴注射疗法治疗效果优于临床公认有效的生物反馈疗法,该方法操作简便,安全性高,值得临床推广应用。[Objective] To investigate the effect of injecting BL34 for levator ani syndrome. [Methods] 80 patients were randomly divided into treatment group （n=40） and control group（n=40）. The treatment group adopted injecting BL34,control group using biofeedback therapy. To assess pain scores, life quality assessment indicators, anal canal pressure before and after the treatment. To compare the clinical efficiency and the recurrence rate in follow-up of 6 months. [Results] The two groups of patients in pain scores, anal resting pressure, anal sphincter pressure, after treatment than before treatment were improved to varying degrees, the treatment group improved significantly better than the control group（P〈0.05）.In physiological function（PF）, body pain（BP） and social fnnction（SF） of life quality assessment indicators, the treatment group improved significantly better than the control group（PF, SF, P〈0.05;BP, P〈 0.01）.In the treatment group, the clinical efficiency was 87.50%, while the control group was 75.00%, two groups were statistically significant（P〈0.05）. Followed up for 6 months, the recurrence rate of the treatment group and the control group was 15.00% and 27.50% respectively, the two groups had no significant difference. [Conclusion] The therapy of injecting BL34 is superior to biofeedback therapy which is clinically recognized effective. The method has the advantages of simple operation, high safety; it is worth of clinical application.
