摘  要:目的 探讨基于营卫理论择时顺势足浴调护失眠患者睡眠质量的效果。方法 选取符合纳入标准的失眠患者95例,随机分为对照组31例、时辰组32例、时辰分方组32例。对照组分别于晚上临睡前及上午任意时段实施桂枝汤中药足浴;时辰组分别于卯时(5:00~7:00)、酉时(17:00~19:00)实施桂枝汤中药足浴;时辰分方组于卯时采用桂枝汤加桂枝柴胡足浴晨方,酉时采用桂枝汤加白芍龙骨牡蛎足浴晚方。三组均每日足浴2次,连续调护4周后采用匹兹堡睡眠质量指数量表(PSQI)评价睡眠质量改善情况,采用《中药新药临床研究指导原则》评价临床疗效。结果 三组患者干预后第4周的PSQI总分、各维度得分(除睡眠障碍外)及临床疗效差异有统计学意义(均P〈0.01);其中PSQI总分、各维度得分(除睡眠障碍外)及临床疗效,时辰分方组显著优于时辰组、对照组,时辰组显著优于对照组(均P〈0.05)。结论 基于营卫理论择时顺势足浴法可有效调护失眠,显著改善患者睡眠质量。Objective To evaluate the effect of Nutritive-Defensive Qi theory guided Chinese herbal foot bath in improving sleep quality of patients with insomnia. Methods A total of 95 patients with insomnia were recruited and randomized into three groups. The control group (n=31) was required to perform foot bath with Chinese herbal Guizhi (Cassia Twig) decoction before going to bed and at anytime in the morning. The experimental group 1 (n= 32) was asked to perform foot bath with same decoction at 5:00-7:00 and 17:00-19:00. The experimental group 2 (n= 32) received foot bath with different herbal medicines at different time. Guizhi and Chaihu (Radix Bupleuri) were used at 5 :00-7100, while Guizhi, Baishao (Radix Paeoniae Alba), Longgu (Os Draconis) and Concha Ostreae were used at 17:00-19:00. All groups performed foot bath twice a day. After four weeks' intervention, the Pittsburgh Sleep Quality Index scale (PSQI) was used to measure sleep quality and the Chinese traditional medicine clinical research guideline was employed to evaluate clinical efficacy. Results The total PSQI score and subscale scores except sleep disorder, as well as clinical efficacy, had significant differences among the three groups (P〈0.01 for all), with the experimental group 2 better than the other two groups and experimental group 1 better than the control group (P〈0.05 for all). Conclusion Nutritive-Defensive Qi theory guided Chinese herbal foot bath can effectively improve sleep quality of patients with insomnia.
