穴位艾灸治疗食管癌贲门癌术后胃瘫综合征的临床研究

摘　　要：目的评价穴位艾灸治疗食管癌贲门癌术后PGS的临床研究。方法对2012年8月—2015年8月南阳医学高等专科学校第二附属医院收治符合食管癌贲门癌术后PGS诊断标准的患者60例进行研究,采用单盲对照法随机分为治疗组30例和对照组30例。将2组患者治疗第1、7、14、21天的胃瘫主要症状指数(GCSI,由症状分级量化评分表进行症状量化)评分,胃管胃液引流量、胃动素数值,建立档案。治疗组:应用常规治疗基础上联合穴位艾灸(取神阙、关元、气海、中脘)进行治疗,穴位艾灸2次/天,早晚各1次,每次30 min,连续7天为一个疗程,应用3个疗程。对照组:采用常规治疗,7天为1个疗程,应用3个疗程。结果 2组在治疗后第7、14、21天GCSI评分,胃管胃液引流量、胃动素数值均有显著变化;检验治疗组与对照组在第7、14、21天GCSI评分,胃管胃液引流量、胃动素数值,治疗组优于对照组,差异有显著性(P<0.05);在临床疗效方面,治疗后结果进行统计学检验,2组治疗后第7、14天痊愈、显效、有效、无效差异均有显著性(P<0.05),第21天2组检验无显著差异(χ2=5.68 P=0.13),但检验2组痊愈人数与非痊愈人数,2组差异有显著性(χ2=4.59 P=0.03)。治疗组总有效率100%,对照组总有效率83.00%,2组总有效率检验差异有显著性(P<0.05)。治疗组与对照组均能有效治疗食管癌贲门癌术后PGS,治疗组相较于对照组更有效。结论穴位艾灸治疗食管癌贲门癌术后PGS,具有标本兼治突出的优势。Objective To evaluate clinical study of acupoints moxa-moxibustion in the treatment of postoperative PGS of esophageal cardia cancer. Methods From August 2012 to August 2015 in the Second Affiliated Hospital of Nanyang Medical College, 60 cases of patients with postoperative diagnostic criteria of PGS were studied, using single blind controlled method, and they were randomly divided into treatment group of 30 cases and control group of 30 cases. At the treatment of 1, 7, 14 and 21 days, gastric paralysis main symptom index（GCSI, by quantitative classification of quantitative assessment of symptom disease） score, stomach tube of gastric juice and gastric prime value were establish archives. Based on conventional treatment, the treatment group applied acupoints moxamoxibustion（ Shenque（ RN8）, Guanyuan（ RN4）, Qihai（ RN6）, Zhongwan（ RN12）） treatment, 2 times/day at morning and night, once every 30 minutes, seven days for a period of treatment, 3 period of treatment. The control group used the conventional treatment, 7 days was a period of treatment, and the treatment continued 3 periods of treatment. Results After treatment of 7, 14 and21 days, the GCSI score, stomach tube of gastric juice and gastric prime value had significant changes. The GCSI score of the treatment group and the control group at 7, 14 and 21 days was inspected. The stomach tube of gastric juice and gastric prime value of the treatment group were better than those of the control group, and there was significant difference（P〈0.05）. In terms of clinical curative effect, for the statistical test results after treatment, after treatment of 7 and14 days, the cure, powerfully, effectively and invalid differences were significant（P〈0.05）. At the inspection of 21 days, there was no significant difference between the two groups（χ2=5.68, P=0.13）, but the numbers of two groups of healing and recovery were significant differences between the two groups（χ2=4.59,P=0.03）.
