颈椎前路零切迹融合器固定系统治疗颈椎间盘突出症疗效分析

摘　　要：目的探讨使用颈椎前路零切迹椎间融合固定系统手术治疗颈椎间盘突出症的临床疗效及优越性。方法2013—03—2015—06，应用颈椎前路零切迹椎间融合固定系统手术治疗24例颈椎间盘突出症患者，男14例，女10例，年龄35—72岁，平均56．5岁。观察患者术后是否出现吞咽困难、植骨融合情况，分别在术前、术后3个月与半年采用JOA评分对手术疗效进行评定。结果所有患者均获得随访，术中出血量为70—120ml，平均（85．3±14．2）ml，手术时间70—120min，平均（90．0±12．8）min，JOA评分由术前的5．6-10．1分上升至术后3个月的14—18．6分及术后半年的14．7～18．6分；所有患者在术后3个月吞咽困难症状完全消失，术后半年均获植骨融合。结论应用颈椎前路零切迹椎间融合固定系统手术治疗颈椎间盘突出症，无需使用颈椎前路钢板，术中操作简便，术后临床效果良好，吞咽困难发生率低，生物力学稳定性良好，植骨融合率高，并发症少。Objective To investigate the clinical efficacy and superiority of fixation for the treatment of protrusion of cervical intervertebral disc with anterior cervical interbody fusion zero profile. Methods From March 2013 to June 2015, cervical anterior interbody fusion fixation system operation was used to treat 24 patients with cervical vertebra disease, 14 patients were male, 10 patients were female, aged from 35 to 72 years old, average 56.5 years. To observe the patients＇ postoperative dysphagia, bone graft fusion and 3 months, six months after the operation and the JOA score was used to assess the efficacy of surgery. Results All patients were followed up. The amount of bleeding during the operation was 70-120 ml, average （85.3±14.2）ml. The operation time was 70-120 min, average（90 ±12.8） min. JOA score was increased from 5.6-10.1 points before operation to 14-18.6 in 3 months after operation, and 14.7±18.6 in 6 months. All patients in 3 months after the swallowing difficult symptoms completely disappeared, 6 months after surgery, all patients had bone graft fusion. Conclusion Zero profile application of anterior cervical interbody fusion fixation surgery in the treatment of cervical spondylosis, without the use of anterior cervical plate, the operation is simple, the postoperative clinical effect is good, dysphagia occurs with low, good biomechanical stability, high rate of bone graft fusion and less complications.
