摘  要:目的探讨2型糖尿病患者血清同型半胱氨酸水平与认知功能障碍之间的关系。方法选择2型糖尿病(T2DM)患者60例为研究对象,根据血清同型半胱氨酸(Hcy)水平是否正常分两组:血清同型半胱氨酸正常为2型糖尿病组1,血清同型半胱氨酸异常为2型糖尿病组2。健康体检者30例为对照组。选用简易智能精神状态检查量表(MMSE)及蒙特利尔认知评估(MoCA)对研究对象及对照组进行认知功能评估。结果 2型糖尿病患者血清同型半胱氨酸水平较对照组明显增加(P<0.05),2型糖尿病患者MMSE评分及MoCA评分均与血清同型半胱氨酸水平均呈负相关(P均<0.05)。2型糖尿病患者与健康对照比较在注意和计算力、记忆力、语言,执行方面功均存在功能障碍(P均<0.05),合并高同型半胱氨酸患者还存在物体命名功能障碍(P<0.05)。结论 2型糖尿病患者血清同型半胱氨酸水平明显增加,且与认知功能障碍程度呈负相关。Mo CA在早期发现认知障碍方面较MMSE更加敏感。2型糖尿病合并高同型半胱氨酸患者除注意和计算力、记忆力、语言,执行方面功能均存在功能障碍,还存在物体命名功能障碍。Objective To investigate the correlation between homocysteine levels of type 2 diabetic patients and cognitive dysfunction.Methods 60 patients with Type 2 diabetes mellitus(T2DM) were enrolled in the study,according to serum homocysteine( Hcy) levels they were divided into two subgroups: normal serum homocysteine group,abnormal serum homocysteine group. 30 cases those who went for health examination were enrolled in the control group. Mini- mental- state examination( MMSE) and Montreal Cognitive Assessment( Mo CA) were used to carry out cognitive function assessment. Results Homocysteine levels in type 2 diabetic patients significantly higher than the control group( P〈0.05). Mo CA and MMSE score were negatively correlated with serum homocysteine levels( P〈0.05). Type 2 diabetes patients present dysfunction in comparison attention and computing power,memory,language,the implementation of the work compared with healthy controls( P〈0.05). Type 2 diabetes patients with homocysteine had dysfunction of name object( P〈0. 05). Conclusions The serum homocysteine levels in diabetic type 2 increased significantly and negatively correlated with the degree of cognitive dysfunction. Mo CA is more sensitive than the MMSE in early detect cognitive obstacles. Type 2 diabetes patients with high homocysteine not only have dysfunction in attention and computing power,memory,language,implementation of the functions are present dysfunction,but also have name object dysfunction.
