高龄股骨颈骨折手术患者腰硬联合麻醉中应用罗哌卡因和布比卡因疗效差异

摘　　要：目的探讨高龄股骨颈骨折手术患者腰硬联合麻醉中应用罗哌卡因和布比卡因疗效差异。方法选取2014年2月至2015年10月我院收治行手术治疗高龄股骨颈骨折患者60例为研究对象,根据患者麻醉中采用麻醉药物不同分为罗哌卡因组(R组)和布比卡因组(B组),观察并记录两组患者运动阻滞起效时间、感觉阻滞起效时间、Bromage评分、感觉恢复时间和并发症发生情况。结果 R组患者运动阻滞起效时间、感觉阻滞起效时间长于B组,Bromage评分低于B组(P<0.05)。R组患者低血压、心动过缓及恶心呕吐发生率明显低于B组(P<0.05)。结论罗哌卡因用于高龄股骨颈骨折手术患者腰硬联合麻醉的效果和安全性优于布比卡因。Objective To compare the anesthesia effect of ropivacaine and bupivacaine in combined spinal and epidural anesthesia for elder patients suffering femoral neck fractures. Methods Sixty elder patients,be admitted in our hospital because of femoral neck fractures during February 2014 and October 2015,were involved in this study. All patients were randomly divided into 2 groups（n=30 in each group） ： ropivacaine group（ group R） and bupivacaine group（ group B）. Sensory block onset time,motor block onset time,Bromage scores,the recovery time of feeling and adverse reactions were compared between the two groups. Results Sensory block onset time and motor block onset time of group R was longer than that in group B,however,the Bromage score was significantly decreased（P〈0.05）. Hemodynamic effects of ropivacaine（ hypotension,bradycardia and nausea and vomiting） were significantly slighter than those of bupivacaine group（P〈0.05）.Conclusions For elder patients suffering femoral neck fractures,ropivacaine may provide superior anesthesia effect than bupivacaine.
