摘　　要：通过互联网技术、搭建数据共享平台、组建专业医疗团队、定期随访、转诊等方式建立以患者、社区和医院为主体的糖尿病全过程管理系统,制定管理行为规范,并在重庆市渝中区进行应用示范,提高对糖尿病患者的管理水平。基于数据共享平台的糖尿病管理系统能解决慢病管理过程中医患脱节问题,通过该系统医生可定期随访了解病人病情,病人也可及时咨询医生。通过组建专业医疗团队,提升社区医生医技水平,为病人提供更专业的指导与帮助,进而提高糖尿病的管理水平。通过示范推广基于数据共享平台的糖尿病管理系统,可以高效地全程管理糖尿病患者,更准确及时地跟踪管理糖尿病患者。Through Internet technology, building data sharing platform, forming professional medical team, regular follow-up, patient referral, establishing the entire process of diabetes management system whose main body is patient, hospital and community. Developing the managerial behavior standard, and then demonstrating the application, which can improve diabetes management. It can solve the disconnect between doctors and patients through the entire process of diabetes management system which based on data sharing platform. Doctor can understand the disease through regular follow-up in this system, and patients can also consult a doctor promptly. It can improve community physician medical technology level through forming professional medical team, which can provide patients with more professional guidance and help, and further improve management of diabetes. It can efficiently manage diabetic, and more accurate and timely track and manage patients with diabetes through demonstration and extension this system.
