摘　　要：目的探讨高同型半胱氨酸血症对高血压前期人群早期肾功能的影响。方法方便选取该院自2014年8月—2015年12月诊治的高血压前期169例为研究组,血压正常57名为对照组(NC组)。根据是否合并高同型半胱氨酸血症(HHcy),研究组分为同型半胱氨酸正常研究组(NHHcy组,n=107)和高同型半胱氨酸血症研究组(HHcy组,n=62)。测定所有受试者同型半胱氨酸(Hcy)、尿微量白蛋白/肌酐(UACR)和肾小球滤过率(GFR)等指标,并比较各组上述指标的差异,分析高同型半胱氨酸血症(HHcy)对高血压前期人群UACR、GFR等肾功能指标的影响。结果研究组HHcy检出率及血压均高于NC组(P<0.05)。与NC组、NHHcy组比较,HHcy组Hcy、UACR明显升高,GFR明显降低,差异均具有统计学意义(P<0.05)。相关性分析显示Hcy与UACR呈正相关(r=0.485,P<0.01),与GFR呈负相关(r=-0.348,P<0.01)。结论合并HHcy高血压前期人群存在早期肾功能损伤及血压升高,检测Hcy可能有利于早期干预靶器官损害和血压进展。Objective To investigate effects of hyperhomocysteinemia to early renal function in prehypertention.Methods Convenient selection 169 prehypertensive patients and 57 normotensive control subjects(NC group) were evaluated. All prehypertensives were divided into 2 groups: prehypertensives with hyperhomocysteinemia(HHcy)(HHcy group, n=107) and prehypertensives without hyperhomocysteinemia(NHHcy group, n =62). Homocysteine(Hcy), albumin/creatinine ratio(UACR), and glomerular filtration rate(GFR) were assessed in all subjects. And compared the differences of above indicators between all groups, and analyzed effects of hyperhomocysteinemia to early renal index UACR and GFR in prehypertention Results Hypertensive patients had the higher prevalence of HHcy and blood pressure than NC group. Compared with NC group and NHHcy group, improved Hcy and UACR while reduced GFR were found in HHcy group significantly(P <0.05). Finally, correlation analysis showed that Hcy was positive associated with UACR(r=0.485,P<0.01), while was negative associated with GFR(r =-0.348, P <0.01).Conclusion Prehypertensive patients with HHcy had early damage of renal function and higher blood pressure. Increased Hcy can be used as a alternative target for preventing target organ damage and delaying the process of blood pressure.
