摘  要:目的探讨胰腺炎大鼠腹腔引流术诱发肺组织内NF-κB和TNF-α的表达变化及丙泊酚的干预效应研究。方法80只SD雄性大鼠随机分为假手术组(sham组),胰腺炎组(SAP组);胰腺炎+腹腔引流组(引流组),胰腺炎+腹腔引流+丙泊酚干预组(丙泊酚组),每组20只。术后12 h观察各组肺组织病理情况,检测血浆以及肺组织中NF-κB和TNF-α情况。结果 sham组0 h、12 h肺组织病理评分低于其他各组(P<0.05)。引流组、丙泊酚组12 h时的病理评分低于SAP组(P<0.05)。引流组、丙泊酚组12 h时血清及肺组织中的NF-κB和TNF-α均低于SAP组(P<0.05)。丙泊酚组12 h时血清及肺组织中的NF-κB和TNF-α均低于引流组(P<0.05)。引流组和丙泊酚组的血清及肺组织中的NF-κB和TNF-α均低于SAP组(P<0.05)。结论丙泊酚干预能够降低胰腺炎大鼠肺组织内NF-κB和TNF-α的表达水平,提高腹腔镜引流治疗胰腺炎效果,其作用机制可能与其减少了炎症因子释放有关。Objective To investigate the expression of NF-κB and TNF-α in lung tissues of rats with acute pancreatitis and the effect of propofol on the changes. Methods Eighty SD male rats were randomly divided into sham group,severe acute pancreatitis group( SAP group),SAP + abdominal drainage group( drainage group),SAP + abdominal drainage + propofol group( propofol group)( n = 20 in each group). Pathological changes of lung tissues were observed at 12 h after surgery,and the expression of NF-κB and TNF-α in plasma and lung tissues was detected. Results The pathological scores at 0 h and 12 h were higher in SAP group,drainage group and propofol group than that in sham group( P〈0. 05). The pathological score in drainage group and propofol group was lower than in SAP group at 12 h( P〈0. 05). In drainage group and propofol group,the NF-κB and TNF-α levels in serum were lower than those in SAP group( P〈0. 05). In propofol group,the NF-κB and TNF-α levels in serum were lower than those in drainage group( P〈0. 05). The TNF-α and NF-κB levels in serum and lung tissues in drainage group and propofol group were lower than that in SAP group( P〈0. 05).Conclusion Propofol can improve the laparoscopic drainage effect in the treatment of pancreatitis and reduce the expression of NF-k B and TNF-a in lung tissues of rats with acute pancreatitis,which may be related to the decrease of release of inflammatory factors.
