体外受精-胚胎移植重复周期的临床结局分析

摘　　要：目的探讨体外受精-胚胎移植重复周期的次数对患者临床结局影响及其原因,研究反复体外受精-胚胎移植失败的相关因素。方法选取2015年1~12月在本院生殖中心接受体外受精-胚胎移植治疗的469例患者作为研究对象,按重复次数将其分为A组（326例）和B组（143例）。A组为重复周期次数为1次的患者,B组为重复周期次数≥2次的患者。比较两组的年龄、不孕年限、HCG日内膜厚度、Gn用量、卵泡数、获卵数、卵裂率、优质胚胎率、临床妊娠率。结果 A组的年龄和Gn用量小于B组,差异有统计学意义（P〈0.05）。A组的卵泡数、获卵数、临床妊娠率高于B组,差异有统计学意义（P〈0.05）。A组的不孕年限、HCG日内膜厚度与B组比较,差异无统计学意义（P〉0.05）。两组的卵裂率和优质胚胎率比较,差异无统计学意义（P〉0.05）。结论在不同重复次数的周期中,年龄的差异直接影响临床结局,适当改善Gn用量,优化胚胎实验室的环境和技术可能会改善反复体外受精-胚胎移植失败患者的临床结局。Objective To explore the influence of the frequency of the repeated cycles of in vitro fertilization and embryo transfer on the clinical outcome of the patients and its causes,to study the related factors of failure of recurrent in vitro fertilization and embryo transfer.Methods 469 cases who received in vitro fertilization and embryo transfer in the reproductive center of our hospital from January to December in 2015 were selected and divided into the group A（326 cases） and group B（143 cases） according to the repeat frequency.Group A was repeated cycle 1,while group B was repeated cycle greater than or equal to 2.The age,duration of infertility,HCG thickness of the endometrium,GN dosage,the number of follicles,oocytes and cleavage rate,high quality embryo rate and clinical pregnancy rate was compared between two groups.Results The age and the amount of Gn in group A was less than that group B,with significant difference（P〈0.05）.The number of follicles,the number of eggs,the clinical pregnancy rate in group A was higher than that oin group B,with significant difference（P〈0.05）.There was no significant difference in the duration of infertility,HCG thickness of the endometrium between group A and group B（P〉0.05）.There was no significant difference in the oocytes and cleavage rate,high quality embryo rate between the two groups（P〉0.05）.Conclusion In the cycle of different repetition frequency,the difference of age directly affects the clinical outcome,The clinical pregnancy outcome of repeated in vitro fertilization and embryo transfer failure in patients may be improved by proper improving the amount of Gn,optimizing the environment and technology of the embryo lab.
