握力圈抓握训练在肿瘤P ICC置管患者中的应用效果

摘　　要：目的：探讨握力圈抓握训练在PICC置管肿瘤患者中的应用效果。方法选取肿瘤PICC患者120例,随机分为观察组57例和对照组63例,对照组给予常规导管护理及指导,观察组在此基础上,在有节奏的背景音乐中,采取包括掌握、五指握、单指握、交握及转圈五组动作；观察两组带管期间静脉炎、不良事件发生率,医疗费用及带管适应度。结果观察组患者静脉炎发生率为5.26％,明显低于对照组19.04％,组间比较差异有统计学意义（Z＝2.171,P〈0.05）；观察组堵管发生率为0.00％,带管期间人均费用为（46.50±22.29）元,明显低于对照组[9.52％,（72.32±30.18）元],差异有统计学意义（P〈0.05）；观察组与对照组比较,带管适应度明显较高（ Z＝－3.125,P〈0.05）。结论握力圈抓握训练可有效预防静脉炎及带管不良事件的发生,降低医疗成本,提高患者的带管适应度。Objective To explore the application effects of the grip ring training on tumor patients with PICC.Methods 120 tumor patients with PICC were investigated and randomly divided into the intervention group （ n=57） and the control group （ n=63） . In the rhythm background music, the intervention group received additional grip training including palm grasp, fingers grasp, single-finger grasp, cross grasp, and turn around based on the routine nursing care. The incidence of phlebitis and adverse events, medical costs, and tube adaptation were compared between the two groups.Results The incidence of phlebitis for the intervention group was significantly lower than the incidence rate for the control group （5.26% vs 19.04%; Z=2.171, P〈0.05）. The incidence rate of tube blocking and the medical costs for the intervention group were also better than the control group [0.00% vs 9.52%;（46.50±22.29） vs （72.32±30.18）; P〈0.05]. Furthermore, the patients in the intervention group performed better tube adaptation compared to the patients in the control group （ Z=-3.125, P〈0. 05）.Conclusions The grip ring training can effectively prevent phlebitis and adverse events such as the tube blocking. It can also reduce the medical costs and improve patients′tube adaptation.
